摘要
目的观察针灸结合手法整复治疗成人寰枢关节半脱位的临床疗效.方法选取广西壮族自治区贵港市人民医院2018年1月—2019年12月间收治的181例成人寰枢关节半脱位患者为研究对象,依据就诊顺序分组,以2018年12月31日为时间节点,在节点前的89例患者采取手法整复治疗设置为对照组,在节点后的另92例患者采取针灸结合手法整复治疗设为实验组,并对患者进行6个月的有效随访.对比两组患者治疗前后的疼痛评分、眩晕评分,临床治疗疗效及随访期间复发的情况.结果两组患者治疗前VAS、DHI评分相比,差异无统计学意义(P>0.05).治疗后两组患者VAS、DHI评分明显下降,其中实验组患者VAS、DHI评分明显低于对照组,差异有统计学意义(P<0.05).实验组患者治疗总有效率(96.74%)高于对照组(83.15%),差异有统计学意义(P<0.05).实验组患者随访期间复发率(3.26%)明显低于对照组(21.43%),差异有统计学意义(P<0.05).结论在成人寰枢关节半脱位的临床治疗中,采取针灸结合手法整复治疗,可以明显缓解疼痛与眩晕情况,提高临床疗效,降低随访复发率,效果理想.
Objective To observe the clinical effect of acupuncture and moxibustion combined with manipulation on adult atlantoaxial subluxation.Methods 181 adult patients with atlantoaxial subluxation admitted to Guigang People's Hospital from January 2018 to December 2019 were selected as the research objects,and they were grouped according to the order of admission,with December 31,2018 as the time node.89 patients before the node were treated with manual rehabilitation and set as the control group,and the other 92 patients after the node were treated with acupuncture and moxibustion combined with manual rehabilitation and set as the experimental group,and the patients were effectively followed up for 6 months.The pain scores and dizziness scores before and after treatment,clinical treatment efficacy,and recurrence during follow-up were compared between the two groups.Results The differences in VAS and DHI scores of the two groups of patients before treatment were not statistically significant(P>0.05).After treatment,the VAS and DHI scores of the two groups of patients decreased significantly,and the VAS and DHI scores of the experimental group were significantly lower than those of the control group,the differences were statistically significant(P<0.05).The total effective rate of treatment in the experimental group(96.74%)was higher than that in the control group(83.15%),the difference was statistically significant(P<0.05).The recurrence rate of the experimental group during follow-up(3.26%)was significantly lower than that of the control group(21.43%),the difference was statistically significant(P<0.05).Conclusion In the clinical treatment of adult atlantoaxial subluxation,acupuncture and moxibustion combined with manual rehabilitation can significantly relieve the pain and dizziness,improve clinical efficacy,and reduce follow-up recurrence rate.The effect is ideal.
