摘要
目的 探讨MMP-9在下肢动脉硬化闭塞症介入治疗前后的动态变化及与术后再狭窄的相关性.方法 纳入2016年2月至2018年5月血管外科收治的76例下肢动脉硬化闭塞症行介入治疗的患者为观察对象,通过动态监测患者血清MMP-9水平,了解患者术后3个月再狭窄的发生情况,并将对术后再狭窄发生的影响因素通过Logistic回归进行分析探讨.结果 术后3个月,76例患者中,66例(86.84%)未出现再狭窄,10例(13.16%)发生了再狭窄;患者MMP-9水平在术后开始快速上升,至术后1周达到高峰,然后开始下降,至术后3个月回归正常水平(F=1313.19,P<0.001).再狭窄患者与非再狭窄患者的血清MMP-9水平动态对比,结果显示再狭窄组患者MMP-9水平各时间点均高于非再狭窄组(P<0.05).对术后发生再狭窄的影响因素进行单因素分析,结果显示术后再狭窄与2型糖尿病以及MMP-9水平上升相关(P<0.05);与性别、心脏病、高血压、吸烟等无明显相关(P>0.05).对单因素分析中有意义的项目进行多因素Logistic回归分析,结果显示2型糖尿病以及MMP-9水平升高是造成术后发生再狭窄的独立危险因素(P<0.05).结论 对下肢动脉硬化闭塞症患者行介入治疗后进行血清MMP-9的动态监测,可对其术后再发动脉狭窄产生重要价值,因此,可以通过控制血清MMP-9水平来达到降低术中再狭窄情况的发生率.
Objective To investigate the dynamic changes of MMP-9 in arteriosclerosis obliterans of lower extremities before and after interventional therapy and its correlation with postoperative restenosis.Methods 76 cases of arteriosclerosis obliterans of lower extremities receiving interventional therapy in our hospital from February 2016 to May 2018 were selected.The serum MMP-9 levels were monitored,meanwhile,the incidence rate of the restenosis at postoperative 3 months was recorded,then the factors influencing the postoperative restenosis were investigated through the Logistic regression analysis.Results At postoperative 3 months,66(86.84%)had no restenosis and 10(13.16%)had restenosis.The level of MMP-9 in patients began to rise rapidly after surgery,reached a peak at postoperative 1wk and then began to decline,returning to normal level at postoperative 3 months(F=1313.19,P<0.001).The levels of MMP-9 in the restenosis group were higher than those in the non-restenosis group(P<0.05).Postoperative restenosis was associated with type 2 diabetes and elevated MMP-9 levels(P<0.05),while had no correlation with gender,heart disease,hypertension,smoking,etc.(P>0.05).Multivariate logistic regression analysis showed that type 2 diabetes and elevated MMP-9 levels were independent risk factors for postoperative restenosis(P<0.05).Conclusion Dynamic monitoring of serum MMP-9 in patients with lower extremity arteriosclerosis obliterans after interventional therapy has important value in predicting postoperative restenosis.Therefore,the incidence of restenosis can be reduced by controlling serum MMP-9 levels.
