摘要
目的:探讨机械通气时应用不同水平的呼气末正压(PEEP)对中心静脉压(CVP)的影响。方法选择我院28例行机械通气并监测 CVP的患者,观察患者在PEEP为0、3、6、9、12、15cmH2O6个水平条件下CVP的变化。结果当PEEP>3cmH2O时CVP随着PEEP的逐渐升高而增加,二者呈正相关。以呼气末正压为自变量X,中心静脉压测量值为因变量Y,可计算出直线回归方程Y=9.981+0.485X (r2=0.973)。结论 PEEP(>3cmH2O)对CVP有明显影响,监测 CVP的动态变化对判断血容量的变化和指导补液更有意义。
Objective To evaluate the ef ect of dif erent positive end expiratory pressure(PEEP)on central venous pressure(CVP)in patients with mechanical ventilation.Methods Twenty-eight cases of respiratory failure with mechanical ventilation were enrol ed in this study.Vessel was inserted via subclavian vein or internal jugular vein in every one,then CVP was measured under dif erent PEEP conditions of 0,3,6,9,12,15cmH2O.Results The CVP value got an increase with a rise of PEEP when it is greater than 3 cmH2O.The linear regression equation Y=9.981+0.485X can be got when taking PEEP as independent variable X and CVP as dependent variable Y.Conclusion When PEEP is greater than 3 cmH2O,it has obvious influence on CVP.The dynamic change of CVP has certain significance to value the change of blood volume and guide the rehydration.
