采用卵巢-附件报告与数据系统评价附件肿块的观察者间一致性研究

摘要 目的评价不同医师采用美国放射学会(ACR)卵巢-附件报告与数据系统(O-RADS)对附件肿块进行描述及分类时,观察者间诊断的一致性。方法由2名超声医师在互盲且均不知病理结果和相关病史的情况下,分别对2019年1月至12月间在湖北省妇幼保健院手术的392个附件肿块(353个良性,39个恶性)的超声图像进行回顾性分析,各自独立描述记录,并进行O-RADS分类。采用Fleiss Kappa检验分析2名医师的描述术语及分类结果的一致性。结果O-RADS描述术语方面2名医师对腹水或腹膜结节的一致性极好(Kappa值=0.940,P<0.001),对肿块分类、内壁或外缘形态的一致性好(Kappa值=0.762、0.712,P<0.001、=0.048),对血流评分的一致性中等(Kappa值=0.510,P<0.001),对乳头状突起的一致性弱(Kappa值=0.362,P<0.001);O-RADS分类方面2名医师间一致性好(Kappa值=0.742,P<0.001)。结论ACR O-RADS在不同医师之间诊断一致性好,能较客观地用于附件肿块的诊断,对规范、统一超声报告,减少不同医师之间的解读误差具有一定的意义。 ObjectiveTo evaluate the inter-reviewer agreement(IRA)of the American College of Radiology(ACR)Ovarian-Adnexal Reporting and Data System(O-RADS)in describing and classifying adnexal masses(AMs)among different physicians.MethodsThis retrospective study included 392 AMs(353 benign,39 malignant)collected at the Maternal and Child Health Hospital of Hubei Province between January 2019 and December 2019.Two sonographers who were blind to the history and pathology analyzed all ultrasound images,and then recorded the descriptors and the O-RADS classifications,independently.The Fleiss kappa(κ)test was applied to evaluate the IRA of O-RADS descriptive terms and scoring results.ResultsFor the two sonographers,the IRA of descriptive terms for ascites or peritoneal nodules was very good(κ=0.940,P<0.001),that for AMs category and morphology of inner wall or external contour was good(κ=0.762 and 0.712,P<0.001 and=0.048,respectively),that for vascularity was fair(κ=0.510,P<0.001),and that for papillary projection was poor(κ=0.362,P<0.001).The IRA of the O-RADS classification was good(κ=0.742,P<0.001).ConclusionThe ACR O-RADS classification has good diagnostic consistency among different physicians,and it can be used as an objective method for the diagnosis of AMs and is of great value for standardizing ultrasound report and reducing the interpretation error among different physicians.
