摘要
目的对比研究平片式与填充式腹股沟疝修补术的治疗效果及对术后并发症和疝复发的影响。方法回顾性分析2020年1月至2022年9月,安徽医科大学第一附属医院东城院区(肥东县人民医院)普外科收治的100例腹股沟疝患者的临床资料。依手术方式分为2组,54例患者接受平片无张力疝修补术,为平片组;46例患者接受填充式无张力疝修补术,为填充组,2组均随访3个月。比较2组患者的手术及住院相关指标、术后疼痛情况、炎症反应和氧化应激指标、术后并发症情况。结果2组术中出血量、手术时间、下床活动时间及住院时间比较差异无统计学意义(P>0.05);平片组住院费用明显少于填充组(P<0.05);术后12 h、1 d,平片组疼痛视觉模拟评分(VAS)明显低于填充组(P<0.05);术后1 d,白细胞介素-6、白细胞介素-17、β-内啡肽水平明显低于填充组(P<0.05),平片组总抗氧化能力水平明显高于填充组(P<0.05);2组术后并发症发生率比较差异无统计学意义(P>0.05)。结论平片无张力疝修补术和填充式无张力疝修补术治疗腹股沟疝均有显著效果,但平片无张力疝修补术费用较低,患者术后疼痛较轻,临床需结合实际情况选择合适的术式。
Objective To compare the application effect of plain and filled inguinal hernia repair and its influence on postoperative complications and hernia recurrence.Methods The clinical data of 100 patients with inguinal hernia admitted to the Department of General Surgery,the East District of the First Affiliated Hospital of Anhui Medical University(Feidong People's Hospital)from January 2020 to September 2022 were retrospectively analyzed.54 patients received plain film tension-free hernia repair,and were in the plain film group,and the other 46 patients received filling tension-free hernia repair and were included in the filling group,and both groups were followed up for 3 months.The related indicators of surgery and hospitalization,postoperative pain,inflammatory response and oxidative stress indicators,and postoperative complications were compared between the two groups.Results There were no differences in intraoperative blood loss,surgical time,ambulation time,or hospital stay between the two groups(P>0.05).The hospitalization cost in the plain film group was significantly less than that in the filling group(P<0.05).At 12 hours and 1 day after surgery,visual analogue scale(VAS)scores in the plain film group were significantly lower than those in the filling group(P<0.05).The IL-6,IL-17,and β-EP level in the plain film group at 1 day after surgery were significantly lower than those in the filling group(P<0.05),while the total antioxidant capacity(T-AOC)level was significantly higher than that in the filling group(P<0.05).There were no differences in the incidence rates of postoperative complications between the two groups(P>0.05).Conclusion Both plain film tension-free hernia repair and filling tension-free hernia repair have significant effects in the treatment of inguinal hernia.However,plain film tension-free hernia repair has a lower cost and milder postoperative pain.Therefore,it is necessary to select an appropriate surgery based on the actual situation.
