Acute myocardial ischemia: an independent factor for the elevation of plasma brain natriuretic peptide levels

摘要 目的探讨急性心肌缺血是否引起血浆脑钠素(brainnatriureticpeptide,BNP)水平升高。方法回顾性分析冠心病无心力衰竭病人168例血浆BNP浓度。其中稳定型心绞痛组40例,不稳定型心绞痛组23例,急性心肌梗死组105例。另外,健康体检者为对照组88例。BNP测定采用酶联免疫吸附试验方法。结果稳定型心绞痛组血浆BNP浓度中位数42.3(7.6~491.3)ng/L,与正常对照组25.9(5.1~87.9)ng/L相比,差异有统计学意义(P=0.001)。不稳定型心绞痛组血浆BNP水平142.4(92.4~2617.7)ng/L,急性心肌梗死组126.0(5.5~3615.1)ng/L,均显著高于对照组(P=0.000)和稳定型心绞痛组(P=0.000)。不稳定型心绞痛组BNP水平显著高于急性心肌梗死组(P=0.04)。结论急性心肌缺血是血浆BNP水平升高的重要原因之一,血浆BNP水平检测可能有助于甄别急性冠状动脉综合征与稳定型冠心病。 s: Objectives To determine whether acute myocardial ischemia acts as an independent factor for the elevation of plasma brain natriuretic peptide (BNP) levels in patients with coronary heart disease (CHD). Methods A total of 168 CHD patients without symptoms of heart failure were included in this retrospective study. Among the 168 patients, 40 were with stable angina (SA group), 23 with unstable angina (UA group), and 105 with acute myocardial infarction (AMI group). Eighty-eight healthy subjects were assigned as control group. The plasma concentrations of BNP were measured by enzyme-linked immunoabsorbent assay. Results Statistically significant differences were found in median values of plasma BNP concentrations between SA group and control group [42.3(7.6~491.3)ng/L vs. 25.9(5.1~87.9)ng/L, P=0.001]. Patients with UA or with AMI had markedly increased BNP levels [142.4(92.4~2617.7)ng/L,126.0(5.5~3615.1)ng/L, respectively], compared to the healthy control people (P=0.000) and SA group (P=0.000). Moreover, BNP concentration was higher in UA patients than in AMI patients(P=0.04). Conclusions Acute myocardial ischemia might be an important factor for the elevation of plasma brain natriuretic peptide concentrations. The measurement of brain natriuretic peptide levels might be useful in discriminating patients with acute coronary syndrome and patients with stable coronary heart disease.
