摘要
背景与目的:Survivin蛋白是新近发现的凋亡抑制因子,研究表明Survivin蛋白在多种恶性肿瘤中高表达。本研究旨在探讨Survivin在结肠癌组织中的表达情况及其临床意义。方法:应用免疫组化LSAB法对79例术前无放、化疗的结肠癌标本及其相应的癌旁组织和12例正常结肠组织进行Survivin检测,并结合肿瘤临床病理特征、预后进行分析。结果:癌旁组织及肿瘤组织Survivin阳性率分别为32.9%(26/79)与78.5%(62/79),正常结肠组织无Survivin表达,三者之间的阳性率有显著性差异(P均<0.05)。Survivin的表达与结肠癌患者年龄、性别、血型、血CEA水平、淋巴结转移、分化程度、浸润深度、Dukes分期均无显著性相关(P均>0.05)。以肿瘤细胞Survivin阳性率>25%作为Survivin高表达的标准,则Survivin高表达组结肠癌患者的3年生存率明显低于低表达组(39.96%vs63.56%,P=0.04)。Cox多因素风险模型分析发现上述因素中仅Dukes分期和Survivin是否高表达是影响预后的因素,Wald值分别为24.225和5.504,P值分别为0.000和0.019。结论:Survivin在结肠癌组织表达上调,提示该基因在结肠癌发生发展中起重要作用;Survivin过度表达提示预后不良,可作为判断结肠癌预后的独立指标。
BACKGROUND &OBJECTIVE: Survivin is a newly identified apoptosis in hibitor. Studies indicated that Survivin over-expressed in malignant tumors. Th is study was designed to evaluate the expression of Survivin in colon cancer,and the correlation of its expression to clinical characteristics and prognosis of patients with colon cancer. METHODS: Expression of Survivin protein was investig ated by immunohistochemistry in 12 cases of normal colon tissues,and 79 cases of colon cancer tissues without the history of radiotherapy or chemotherapy. RESUL TS: Expression of Survivin was detected in 78.5%(62/79) of colon cancer tissues ,higher than that in tumor nests adjacent tissues,which was 32.9%(26/79). In co ntrast,normal colon tissues did not express Survivin. There was no relationship between the expression of Survivin and age,sex,blood type,serum CEA level,lymph node metastasis,histological grade,infiltration degree,and Dukesstage(P >0.05) . When 25%was used as the cut-off point between high and low expression of Sur vivin,3-year overall survival rate of low expression group (63.56%) was much h igher than that of high expression group (39.96%)(P=0.04). Cox proportional h azards model analysis showed that Dukesstage, and high expression of Survivin were the only 2 prognosis factors influenced the survival times (Wald values wer e 24.225, and 5.504,respectively,P values were 0.000,and 0.019,respectively). CO NCLUSIONS: There is a high expression level of Survivin in colon cancer,it may p lay an important role in tumorgenesis and development of colon cancer. Over-exp ression of Survivin was related with poor prognosis,it may be a potential progno stic index.
