摘要
目的探讨不同剂量甘露醇对急性脑出血患者脑血流动力学的影响。方法急性脑出血患者30例,首次静脉输注20%甘露醇125ml,15min输注完毕;间隔4~6h静脉再次输注20%甘露醇250ml,30min输注完毕,于输注前及输注后30、60、90、120、180、240min行TCD监测。监测指标包括双侧大脑中动脉收缩期峰值流速、舒张期末流速、平均血流速度、搏动指数、血流速度变化百分率和搏动指数变化百分率。结果①125、250ml甘露醇输注后脑血流速度均升高,30min达到流速的最高峰,随后呈下降趋势;②125ml甘露醇输注后TCD搏动指数在病变侧与输注前比较,差异无统计学意义(P>0.05),非病变侧30~120min下降,与输注前差异有统计学意义(P>0.05);③250ml甘露醇输注后搏动指数在病变侧30~120min下降,非病变侧60~120min下降,与输注前差异均有统计学意义(P>0.05)。结论急性脑出血患者输注甘露醇可改善脑血流速度,降低病变侧(输注250ml甘露醇)和非病变侧(输注125和250ml甘露醇)的搏动指数。
Objective To investigate the effect of different doses of mannitol on the hemodynamics in patients with acute intracerebral hemorrhage.Methods A single dose of 20% mannitol 125 mL was infused intravenously in 30 patients with acute intracerebral hemorrhage,it took about 15 min to finish the infusion;then 20% mannitol 250 mL was infused 4 to 6 hours after the first infusion,and it took about 30 min to finish the infusion.TCD monitoring was performed before and 30,60,90,120,180,240 min after the infusion.The ...
