家族过敏史和粉尘螨对新生儿脐血Th1/Th2细胞因子表达及婴儿期变应性疾病发生的影响 被引量:3

摘要 目的了解家族过敏史和粉尘螨变应原刺激对新生儿脐血Th1/Th2细胞因子表达及婴儿期变应性疾病发生的影响。方法足月新生儿34例,分为有、无家族过敏史2组,每组17例。取其脐静脉血10 mL,经淋巴细胞分离液分离脐血单个核细胞(CBMC),体外经植物血凝素(PHA)或粉尘螨刺激培养48 h,ELISA检测培养上清液中IL-4和IFNγ-表达水平。对2组新生儿每1、2个月以电话或门诊随访1次,为期1 a。结果有、无家族过敏史2组新生儿脐血未经刺激的CBMC培养上清中IL-4水平分别为(11.35±1.80)ng/L和(11.0±1.50)ng/L,IFNγ-水平分别为(9.55±1.47)ng/L和(10.19±1.37)ng/L,2组比较均无显著性差异。经PHA刺激后,IL-4水平分别为(43.45±4.57)ng/L和(37.58±3.41)ng/L,2组比较有显著性差异;IFNγ-水平分别为(72.61±25.40)ng/L和(65.84±29.96)ng/L,2组比较无显著性差异。低水平粉尘螨刺激后,IL-4水平分别为(40.54±3.64)ng/L和(37.17±2.60)ng/L;IFNγ-水平分别为(35.30±2.73)ng/L和(40.55±1.85)ng/L,2组比较均有显著性差异。高水平粉尘螨刺激后,IL-4水平分别为(43.50±3.19)ng/L和(39.55±4.13)ng/L,2组比较有显著性差异;IFNγ-水平分别为(39.40±5.21)ng/L和(40.94±2.96)ng/L,2组比较无显著性差异。对2组新生儿随访观察1 a,除去失访病例,有家族过敏史新生儿13例中7例发生变应性疾病。无家族过敏史新生儿15例中2例发生变应性疾病,2组比较有显著性差异。结论有家族过敏史新生儿脐血Th2相对占优势,粉尘螨变应原刺激使Th2相对优势更为明显。有家族过敏史新生儿日后可能更易患变应性疾病。 Objective To investigate the effects of family allergic history and dermatophagoides farina on the expressions of Th1/Th2 cytokine of cord blood and allergic disorders in infancy.Methods Ten mil cord blood were obtained from 34 neonates which 17 cases had family allergic history and 17 cases didn′t have.Cord blood mononuclear cells(CBMC) were isolated by gradient centrifugation with Ficoll and were cultured with phytohemagglutinin(PHA) or dermatophagoides farina for 48 hours in vitro.The expressions of inte...
