摘要
目的了解福州市社区卫生服务机构对高血压的综合管理现状。方法选取福州市台江区上海街道社区卫生服务中心以及该社区950名高血压患者为研究对象,采用现场观察和负责人深入访谈两种方法进行调查研究。结果福州市台江区上海街道社区卫生服务中心从建立以来就对高血压病人实施专项管理,采取福州社区卫生服务管理系统(F2-CHSS)软件进行计算机管理,每季度入户随访一次,随访结果及时录入个人健康档案。结论该社区实施的高血压管理项目取得了一定的成效,但在综合防治方面还存在一些问题,如管理机制不完善、双向转诊指向不明等,初步总结并提出了一套适合该社区高血压防治的方案。
Objective To investigate the integrated management of hypertension in community health services organizations of Fuzhou.Methods Totally 950 hypertension patients from Fuzhou Shanghai Community Health Center and were enrolled,and site observation and in-depth interviews were taken.Results A special management mode of hypertension including computer management and follow-up management was adopted since the establishment of Fuzhou Shanghai Community Health Center.Conclusion The community has got a certain of achievement in hypertension management.However there exist in comprehensive prevention and treatment still some problems such as imperfect management mechanism and unfixed direction of two-way referral.We propose primarily a program of hypertension management suitable to the community.
