摘要
目的 评价螺旋 CT门静脉血管造影 (CTP)的最佳成像技术及其临床应用价值。 方法 对 6 0例正常和 10 8例疑有肝脏疾病的患者作了最大强度投影 (MIP)和表面遮盖法 (SSD) CTP成像。使用不同的准直、延迟时间及扫描方向将正常组随机分为六组 ,定性和定量分析各组图像的差别。 结果 准直 3.0~ 5 .0 m m ,延迟时间 45~ 5 0 s,自足端方向扫描 MIP CTP能同时显示 4级以上门静脉分支和 2级以上的肝静脉分支。 CTP能直观地评价门静脉和肝静脉的位置、管径、有无门静脉受侵或癌栓形成 ,了解门静脉高压侧支循环的分布范围和程度。 CTP有助于经颈静脉肝内门腔静脉分流术 (TIPS)术前定位、Budd- Chiari综合症的诊断和术后随访。SSD CTP立体感强 ,但不利于细微结构和癌栓的显示。 结论 CTP是门静脉和肝静脉无创性检查的可靠方法 。
Objective To evaluate the technique and clinical application of helical CT portography (CTP). Methods The maximum intensity projection (MIP) and surface shade display (SSD) CTP images were performed in 60 normal subjects and 108 patients suspected of hepatic diseases. The normal subjects were randomly divided into 6 groups according to the different collimation, delay time and scan direction, The image quality was evaluated using qualitative and quantitative analysis. Results By using collimation 3.0~5.0 mm, delay time 45~50 seconds with the scan direction from feet to head, MIP CTP image could display more than 4 grade branches of portal veins and 2 grade branches of hepatic veins. CTP image could directly demonstrate the location and width of the portal and hepatic veins, detect the invasion and tumor thrombosis of portal vein, and it also could show the range of collateral circulation and estimate its degree. CTP was useful in the preoperative planning of transjular intrahepatic portosystemic shunt, the diagnosis and follow up of Budd Chiari syndrome. SSD CTP image could provide higher three dimensional relationship, but it was difficult to display the small vessel structures and tumor thrombosis. Conclusions As a noninvasive and reliable method, CTP can provide valuable information about the portal venous and hepatic venous system.
