The effect of High-Flux hemodialysis in uremic patients

摘要 目的 观察高通量血液透析(HPD)和常规血液透析(CHD)对血尿素氮(BUN)、肌酐(Cr)和磷(P)的清除作用及对血脂的影响。方法 将30例规律透析患者随机分为观察组(HPD)和对照组(CHD),各15例。HPD组采用F60滤过器,每周透析3次,每次4小时;CHD组采用F6普通透析器,每周透析3次,每次5小时,对两组疗效进行观察比较。结果 两组患者透析前后BUN、Cr下降率差异不显著(P>0.05),磷下降率有显著性差异(P<0.05);透析一年后HPD组甘油三脂明显下降(P<0.05),CHD组甘油三脂及胆固醇均上升(P<0.01)。结论 HPD对磷的清除优于CHD,对BUN和Cr的清除两组差异不显著,HPD可改善脂代谢。部分病人搔痒症状得到改善,心绞痛发作次数减少。 Objective To observe the clearance of BUN, serum creatinine, blood phosphate and the change of blood lipid with routine hemodialysis and High - Flux hemodialysis respectively. Methods 30 he-modialysis patients were divided into two groups randomly. F60 dialyzers were used in one group with three times treatments per week and 4 hours per treatment. F6 dialyzers were used in the other group with 5 hours per treatment. Resuits The clearance of BUN, serum creatinine was not found difference in two groups(P> 0.05) . The clearance of blood phosphate in High- Flux hemodialysis patients was better than that of routine hemodialysis( P < 0.05) . After one year, plasma triglyceride was decreased obviously in High - Flux hemodi-alysis patients; Plasma triglyceride and cholesterol were increased obviously in routine hemodialysis patients (P < 0.05, P < 0.01, respectively). Conclusions The clearance of phosphate in High - Flux hemodialysis was better than in routine hemodialysis and there was no difference in the clearance of BUN and serum creatinine. High - Flux hemodialysis can improve the metabolism of blood lipid.
