脉压对老年患者冠脉病变严重程度的预测价值

The value of pulse pressure for the severity of coronary heart disease in elderly

摘要 目的探讨老年患者脉压与冠脉病变严重程度之间的关系。方法 268例行冠脉造影(CAG)检查的老年患者,按其临床表现和CAG的结果分为冠心病(CHD)组[包括不稳定性心绞痛(UAP)组、稳定性心绞痛(SAP)组]和对照组,采用袖带加压法连续测量患者入院后三天的晨起收缩压、舒张压,并计算脉压。依据Gensini评分以及冠状动脉病变的血管支数对冠脉病变严重程度进行评估,探讨其与血压不同成分之间关系。结果 CHD组收缩压和脉压水平明显高于对照组(P<0.05);单支病变组、双支病变组、三支病变组脉压均明显高于对照组(P<0.05),且随病变支数的增多,脉压水平逐渐增高,差异具有统计学意义(P<0.05);UAP组收缩压、脉压均高于对照组和SAP组(P均<0.05)。脉压水平与Gensini评分正相关(r=0.199,P<0.01)。结论脉压较收缩压、舒张压更能反映大动脉弹性,不仅与CHD及冠脉病变的严重程度关系密切,还可能与冠状动脉粥样斑块的不稳定性有关。 Object ive To explore the relat ionship of the pulse pressure and the severity of coronary artery disease in elderly.Methods Choosed 268 pat ients as the objects of study who received coronany angiography(CAG).According to the clinical feature and the resul t of CAG,all the cases were divide into coronary heart disease(CHD) [include unstable angina pectoris(UAP) and stable angina pectoris(SAP) groups]and non-CHD groups . Measured systol ic blood pressure and diastolic blood pres sure using sleeve-band pressurizing method in morning once a day in the ��rst three days since being in hospi tal ,furthermore,calculated pulse pressure.The severity of coronary artery disease was evaluated by Gensini score and the number of diseased arteries.The relat ionship between the s everi ty of coronary artery lesion and components of blood pressure was analysed.Resul ts Compared wi th non-CHD group,the levels of PP and SBP were signi��cant higher in CHD group(P<0.05);the level of PP was higher in single、double and multiple pathological lesion groups(P<0.05) and gradually heightened with the increasing number of lesion,the differences were statistically signi��cant compared with non-CHD group(P<0.05). The level of PP was pos itively related to the Gensini score(r=0.199,P<0.01).Conclusions PP was the important parameter which can re��ect the elas ticity of large artery,i t not only closely related to CHD and the severity of pathologic changes in coronary artery branches,but also had connexions with instabil ity of atherosclerotic plaques.
