摘要
目的通过对大鼠缺血后肢皮下注射重组人生长激素(rhGH),观察缺血肢体功能的改善情况及rhGH对缺血组织血管新生情况的影响。方法将75只Wistar大鼠离断右侧股动脉及其分支,制备大鼠后肢缺血模型。采用随机数字法分为五组,A组:单纯后肢缺血模型手术组;B组:0.2mg·kg-1·d-1皮下注射rhGH组;C组:0.4mg·kg-1·d-1皮下注射rhGH组;D组:0.4mg·kg-1·d-1皮下注射rhGH溶剂对照组;E组:空白手术对照组。根据大鼠缺血后肢的受损情况及活动变化,分别在模型建立的术后2d、2周、4周进行功能评分。分别于术后第2周和第4周分两次平均处死实验大鼠,取患肢内收肌制备组织切片,行HE染色,400倍高倍镜下行微血管计数;免疫组织化学法染色检测VEGF的表达情况;按Weidner的方法计算毛细血管密度。结果术后第2天A、B、C、D组大鼠缺血后肢功能情况均较差;术后第4周功能恢复情况为B组优于A和D组、C组明显优于A、B、D组。HE染色微血管计数,A、B、C、D组术后4周高于2周;术后4周,B组高于A和D组,C组明显高于A、B、D组。毛细血管密度检测结果与HE染色的趋势一致。结论 rhGH可以促进血管内皮生长因子(VEGF)在缺血肢体骨骼肌中的表达,从而可能促进缺血肢体的血管再生;通过增加一定剂量的rhGH,可以明显改善缺血肢体的功能恢复。
Objective To observe the improvement of ischemic hindlimbs function and angiogenesis in ischemic tissue situation by subcutaneous injection of recombinant human growth hormone (rhGH) in ischemic hindlimbs. Methods 75 Wistar rats model of hindlimb ischemia were made by transection of the right femoral artery and its branches,and the rats were randomly divided into five groups.A group:a simple model of hindlimb ischemia surgery group;B group:0.2 mg·kg-1·d-1 rhGH subcutaneous injection group;C group:0.4 mg·kg-1·d-1 rhGH subcutaneous injection group;D group:0.4 mg·kg-1·d-1 rhGH solvent subcutaneous injection group;E group:operation control group.According to the damage of ischemic hindlimb and activity changes, function scores were assessed 2 days,2 weeks and 4 weeks after surgery. Respectively,2 weeks and 4 weeks after operation, rats were sacrificed and the limb adductor muscle were biopsied for HE staining and microvessel count.Expression of VEGF was detected by immunohistochemical staining and capillary density was counted according to Weidner method.Results The function of ischemic hindlimb were poor in A, B, C and D group 2 days after surgery.The functional recovery of B group was better than A and D group,C group was significantly better than A,B and D group 4 weeks after surgery.HE staining microvessel counts of 4 weeks after surgery were more than 2 weeks in A,B,C and D group.B group were more than A and D group in 4 weeks,C group were significantly more than A ,B and D group in 4 weeks.Capillary density test results had coincident tendency with HE staining microvessel counts.Conclusions rhGH can promote vascular endothelial growth factor (VEGF) expression in ischemic skeletal muscle,thereby promoting angiogenesis in ischemic limbs.By increasing the dose of rhGH, functional recovery of ischemic limbs can be significantly improved.
