孕20~40周正常胎儿心脏各径线超声心动图测值 被引量:7

Measurement values of the diameters of normal fetal cardiac by fetal echocardiogram in mid-second trimester

摘要 目的测量孕20~40周正常胎儿心脏左心房、右心房、左心室、右心室、主动脉、肺动脉各径线参数,评价其动态变化规律。方法以二维超声对854例正常胎儿心脏各径线进行测量,得出各项参数,按孕周分为五组:20~24周;25~28周;29~32周;33~36周;37~40周。计算每组中各项参数的平均值;每组之间及各参数与孕周均进行相关分析比较。结果胎儿心脏各径线参数均与孕周直线关系较密切(左心室、右心室、左心房、右心房、主动脉内径、肺动脉内径的相关系数r值分别为0.69、0.68、0.70、0.68、0.73、0.72),同时进行相关系数的显著性检验,得出均P<0.05,具有统计学意义。结论胎儿心脏各径线大小与孕周呈明显相关,据此可评价胎儿心脏发育情况,胎儿心脏发育以右心占优势;掌握此孕周范围胎儿心脏各径线正常值,对于更准确的识别胎儿先心病具有很大帮助。 Objective To measure the normal fetal cardiac reference values about left atrium,right atrium,left ventricle,right ventricle,aorta,pulmonary artery in mid-second trimester and evaluate the dynamic changes.Methods The diameters of 854 cases of normal fetal cardiac were measured by two-dimensional ultrasound and the parameters were obtained,according to gestational weeks,the cases were divided into five groups:the first group:20-24 weeks;the second group:25-28 weeks;the third group:29-32 weeks;the fourth group:33-36 weeks;the fifth group:37-40 weeks.Calculated the average of the parameters in each groups and then compared the various parameters with each gestational weeks among every groups by regression correlation analysis.Results The size of the atrium and ventricle,the diameter of the aorta and pulmonary artery were enlarged with increasing gestational weeks,and gestational weeks were significantly positive correlated(P<0.05),the interior diameters of fetal right atrium and ventricle were greater than the left,the interior diameters of pulmonary artery were greater than aortic,with statistical significance.Conclusions The diameters of fetal cardiac size are significantly associated with gestational weeks.According to these,we can evaluate the development situations of fetal cardiac.The development of fetal cardiac is dominant in right cardiac.To grasp the normal values diameters of fetal cardiac during this gestational week range is helpful to identify the fetal congenital heart disease.
