摘要
目的:分析CT对迟发性肺挫伤的诊断价值。材料与方法:105例胸部外伤患者初次胸部CT检查均未见肺挫伤,于伤后1～3天出现不同程度胸痛、咳嗽、呼吸困难后复查CT发现肺部有不同程度迟发性肺挫伤及迟发性血胸;而且还对迟发性肺挫伤与发生时间、发生部位、与以往肺部疾病以及迟发性血胸的相关性进行分析。结果:迟发肺挫伤灶呈形态不规则小条片状23例,粗条索状35例,云絮状20例,类玻璃样新月状27例,多为一侧发生。迟发性肺挫伤大都于伤后1～3天内发生,本文发生迟发性肺挫伤以下肺为多见。结论:CT不但能及时发现迟发性肺挫伤灶,而且还能发现其发生的部位、损伤程度以及并发症,为临床诊断提供了可靠的丰富影像信息;发生在中老年人迟发性肺挫伤灶,吸收较慢、部份遗留肺纤维化、胸膜增厚、黏连。
Purpose:Analysis the diagnostic value of CT for delayed pulmonary contusion.Materials and Methods:105 cases of chest trauma patients,the initial chest CT scan showed no pulmonary contusion,1-3 days after injury with varying degrees of chest pain,cough,difficulty breathing,after with the CT scan found that the lungs have varying degrees of delayed pulmonary contusion and delayed hemothorax;but also use the time of occurrence and location of delayed pulmonary contusion with the previous lung disease and the delayed hemothorax for analysis.Results:There are 23 cases for delayed pulmonary contusion lesions were irregular strip sheet,23 cases for thick cord-like,20 cases for cottony clouds,27 cases for glass-like and Crescent-shaped,and mostly occur by one side.The Delayed Pulmonary Contusion also occurred in 1-3 days after injury,In this article,the Delayed Pulmonary Contusion most occurred on the lower lung.Conclusion:CT can not only timely detection of delayed pulmonary contusion foci,but also found that the site of occurrence,degree of injury and complications,provide a reliable rich image information for clinical diagnosis;delayed pulmonary contusion lesions in elderly patients with absorptionslower part of the legacy pulmonary fibrosis,pleural thickening and adhesion.
