摘要
目的:建立感染微种植体动物模型,观察在200克力作用下不同时间段,微螺钉种植体-骨界面各部位的组织变化。方法:健康雄性杂种犬4只,在犬的双侧上颌尖牙近远中、第二双尖牙近远中颊侧骨板各植入微螺钉种植体一只,共32只。微螺钉种植体颈部缠绕正畸橡皮圈及4-0缝合线。随机选一侧为实验组,一侧为对照组。实验组在两种植体间即刻施加200克的力,对照组不加力。分别于术后1周、2周、4周、8周处死动物。HE染色法观察不同时间段种植体-骨界面的变化。结果:观察期内2枚种植体出现松动、未脱落,1周时种植体周围牙龈粘膜出现明显的红、肿等炎性反应,光镜下出现大量的急慢性炎症细胞,毛细血管增生扩张,有少许新生纤维组织,越接近尖部炎性反应越轻;2周时中部受力侧骨质的连续性破坏;4周颈部见少量的骨质破坏,同时有片状的新生骨形成;8周颈部骨质破坏吸收,对照组骨质破坏较实验组轻。结论:微螺钉种植体在感染状态下,骨质的吸收是由颈部向尖部进行的;加力较不加力的骨质破坏严重。
Objective: The aim of this study was,through the metainfective animal experiment,to observe histological alterations of the different part of the tissue surrounding micro-screw implant loaded with 200G horizontal force for different time(one,two,four,eigth weeks)and to disscuss the healing model of jaw after insertion of microscrew.In the end,hope to give some advice to the clinical operation.Methods: Four male crossbred dogs and thirty-two micro-screw implant.There were inserted into by the seif-driving microscrew in the upper canines vicinity at cuspidal and molar of crossbred dogs respetively,cotton ligatures and rubber were placed around the healing-abutments to make infect.Each two mini-implants were loaded with a horizontal force of 200g immediately by superelastic nickel-titanium coil springs.Four unloaded micro-screw implant in other side served as control group.These dogs were sacrificed at one,two,four,eight weeks in excessive anesthesia respectively.The bone-implant samples were made and were observed under optical microscope after HE stained.Results: Mobility of only two microscrew was observed,but not lost.One weekly,the microscopic features of experimental group were chronic inflammatory changes in the crevices of the implants.Varying degrees of fibrous tissues can be seen.There is no difference in each side.The inflammation was less mild in middle than in crevices of implant.Two weekly,chondroid structures and connective tissues formed in crevices of implants.Osteoclasts can been seen with the destruetion of bone.Four weekly,bone destruetion can be seen in loaded areas.Eight wekly,connective tissues can be seen in middle of experimental group implants.Conclusion: Micro-screw implants in the infection,bone absorption by the neck to the tip of the microscrew implant;Bone destruetion is more severe in loaded group than in non-loaded group.
