摘要
目的评价透光旋切术(TIPP)联合激光、射频及内翻剥脱治疗下肢静脉曲张患者的近远期临床疗效。方法对2818例下肢静脉曲张患者分别采用腔内激光+TIPP,内翻剥脱+TIPP,腔内射频+TIPP联合治疗方法,观察其临床疗效。结果每条肢体透光旋切时间为18～35(平均24)min;手术切口2～6(平均3.5)个;术后住院时间3～11(平均4.8)d。术后皮下血肿54例(1.9%),皮肤感觉异常1128例(40%),蜂窝织炎48例(1.7%),皮肤穿孔14例(0.5%),残留静脉62例(2.2%)。术后1520例患者获得随访,随访时间为1个月至7年,平均3.5年,随访复发23例(1.5%)。结论 TIPP具有创伤小、恢复快、美观等优点,是治疗下肢静脉曲张的一种理想术式。
Objective To evaluate the short and long-term clinical effects of transilluminated powered phlebectomy(TIPP) combined with laser, radio frequency and invaginated vein stripping in treatment of 2818 cases of varicose veins of lower limbs. Methods Endovenous laser therapy + TIPP,invaginated vein stripping + TIPP and endovenous radiofrequency occlusion + TIPP were used and clinical efficacy indicators were observed. Results The time of TIPP of each limb was 18～35( average 24) minutes, with 2～6(average 3.5) surgical incisions. Postoperative hospital stay was 3～11(average 4.8)days. Postoperatively, subcutaneous hematoma,skin paresthesias,cellulitis,skin perforation,residual veins were occurred in 54(1.9%), 1128(40%), 48(1.7%), 14(0.5%) and 62(2.2%) cases, respectively. During a mean follow-up time of 3.5 years (1 month to 7 years),23 (1.5%) cases recurred. Conclusion TIPP with less trauma,quicker recovery and aesthetics is an ideal method to treat varicose veins.
