摘要
目的探讨超声乳化白内障吸除术对皮质性白内障的治疗效果。方法回顾性分析山东青岛市第五人民医院眼科2010年6月至2013年5月收治的120例(240只眼)皮质性白内障患者的临床资料。根据皮质性白内障的不同分型,将60例(120只眼)未成熟型皮质性白内障患者作为白内障未成熟组,60例(120只眼)成熟型皮质性白内障患者作为白内障成熟组,均接受超声乳化白内障吸除术治疗。对比两组患者在手术时间、术后视力及术后并发症等方面的差异。两组患者手术时间和术后视力的比较以均数±标准差(x珋±s)的形式表示,组间比较采用t检验;两组患者术后并发症情况的比较采用卡方检验。结果白内障未成熟组患者手术时间为(28.75±4.32)min,白内障成熟组患者手术时间为(34.58±5.16)min,两组患者手术时间比较,差异无统计学意义(t=6.71,P<0.05)。白内障未成熟组患者术后视力为(0.58±0.12),白内障成熟组患者术后视力为(0.55±0.14),两组患者术后视力比较,差异无统计学意义(t=1.26,P>0.05)。白内障未成熟组患者术后发生角膜水肿者2例(2只眼,1.67%),虹膜炎者4例(4只眼,3.33%),并发症发生率为5.00%;白内障成熟组术后发生角膜水肿者10例(10只眼,8.33%),虹膜炎者8例(8只眼,6.67%),后囊膜破裂者4例(4只眼,3.33%),并发症发生率为18.33%,白内障成熟组手术并发症发生率高于白内障未成熟组,差异有统计学意义(χ2=5.18,P<0.05)。结论超声乳化白内障吸除术治疗皮质性白内障可明显改善患者的视力,但手术难度大,且操作时间长,在临床工作中应予以重视,积极防治手术并发症。
Objective To investigate the effect of phacoemulsification in the treatment of cortical cataract.Methods Medical records of 60 patients (120 eyes) with mature cortical cataract and 60 patients with immature cortical cataract ( 120 eyes ) who underwent phacoemulsification in the Department of Ophthalmology, the Fifth People′s Hospital of Qingdao city, Shandong, China between June 2010 and May 2013 were retrieved.Data on the operative time, postoperative visual acuity, and surgical complications in the two groups were comparatively analyzed by t-test.Results The operative time in the immature cortical cataract group (28.75±4.32)min was significantly shorter than that (34.58±5.16)min in the mature cortical cataract group (t=6.71,P<0.05), but the visual acuity was not significantly different between the immature cortical cataract group (0.58±0.12) and the mature cortical cataract group (0.55±0.14) (t=1.26,P >0.05).Postoperative complications was observed in 3 (5.0%) immature cortical cataract patients (1 case of corneal edema and 2 cases of iritis),but in 11 (18.33%) mature cortical cataract patients (5 cases of corneal edema,4 cases of iritis, and 11 cases of posterior capsular rupture) (χ2=5.18, P <0.05).Conclusions Surgery can significantly improve the visual acuity of both immature and mature cortical cataract patients to the same extent,but the procedure is more time-consuming and associated with a higher rate of complications in mature cortical cataract patients as compared with immature cortical cataract patients.These intriguing features of the phacoemulsification procedure need to be taken into account seriously in the treatment of immature and mature cortical cataracts.
