猪肺磷脂注射液治疗新生儿重症胎粪吸入综合征的疗效观察 被引量:1

摘要 目的:探讨肺表面活性物质(Pulmonary Surfactant,PS)猪肺磷脂注射液治疗新生儿重症胎粪吸入综合征(MAS)的疗效。方法:将本院新生儿监护中心(Neonatal Intensive Care Uint,NICU)2006年7月至2011年6月收治新生儿重症MAS患儿66例随机分为治疗组和对照组,对照组仅采用常规呼吸道护理、纠正酸中毒、呼吸机支持、抗生素抗感染;治疗组在对照组基础上,上机前加用猪肺磷脂注射液100~200mg/kg气管内滴入。比较2组患儿的血气分析pH值、肺氧合功能、机械通气时间、用氧时间、住院时间。结果:治疗组治疗后1、6、12、24、48、72小时动脉血气分析pH值比对照组较快改善,1、12、24小时氧合指数(OI)明显低于对照组(P<0.05);动脉/肺泡氧分压比值(PaO2/PAO2)高于对照组(P<0.05);治疗组机械通气时间、用氧时间和住院时间均显著少于对照组(P<0.05)。结论:PS治疗能有效改善MAS患儿的肺氧合功能,缩短应用机械通气、用氧时间及病程。 [Objective] To explore the efficacy of Poractant Alfa Injection(pulmonary surfactant,PS) in the newborn infants with severe meconium aspiration syndrome(MAS).[Method] 66 infants with severe MAS in NICU from July 2006 to June 2011 were divided into two groups: treated with PS and treated with usual way.The potential of hydrogen(PH) of blood gas analysis of artery and the length of mechanical ventilation,use oxygen and hospitalization were compared between these two groups.[Result] The PH of blood gas analysis of artery in treatment period of 1,6,12,24,48 and 72 hours of the PS group were increased than those of control group;The mean oxygenation(OI) in treatment period of 1,12 and 24 hours of the PS group were lower than those of control group;The mean alveolar oxygen tension ratio(PaO2/PAO2) of the PS group were higher than the control group.The duration of mechanical ventilation,oxygen therapy and hospital stay was significantly shorter in the PS group than those in the control group.[Conclusion] PS therapy may result in a rapid improvement in oxygenation,and reduce the duration of mechanical ventilation and hospitalization time of newborn infants with severe MAS.
