摘要
目的：获取我省国人消化性溃疡的流行病学特征资料。方法：采用同时性、前瞻性调查，自1997年1月至1997年12月止收集新确诊的溃疡病例，记录性别、年龄、职业、溃疡病灶的部位、大小、数量及是否合并HP感染等。结果：共调查11983例消化性溃疡患者，消化性溃疡的年发病率根据标准化人口统计估计为0．084％，男女患病比为3．6：1，从业人员中以管理人员发病率最高，达0．40％，高峰年龄组为30－49岁，十二指肠球部溃疡与胃溃疡的发病比为1．9：1，溃疡大小以＜10mm居多，占80％以上，胃溃疡和十二指肠球部溃疡的好发部位分别是小弯和前壁。幽门螺旋杆菌（HP）感染阳性率为73．8％。结论：消化性溃疡是我省的一种多发疾病，其年发病率估计为0．084％，此病好发于壮年，男性居多，管理人员中发病率最高；我省男性多吸烟是导致男性溃疡多发的主要因素；HP阳性占消化性溃疡的比例数以前有明显下降。
Objective To obtain the epidemiologic data on peptic ulcer disease among Zhejiang people. Methods A simultaneous and prospective investigation was conducted. Patients who were newly diagnosed as peptic ulcer were collected from January 1, 1997 to December 31, 1997 and some items were recorded, such as sex, age, occupation,and location, size and number of ulcers, and Helicobacter pylori (HP) infection. Results Eleven thousand nine hundred and eighty three peptic ulcer cases were investigated. The incidence of peptic ulcer within one year was about 0.084% according to standardized population statistics. The rate of incidence between males and females was 3. 6: 1,and the highest incidence among different occupational groups was 0. 40 % found in administrators. The peak age was during 30 - 49 years old. The rate of gastric/duodenal ulcers was 1: 1. 9. The most common size of both ulcers was between 3 - 10mm representing more than 80. 0% of the total, and the most common location of gastric ulcers was the lesser curvature while duodenal ones occurred in the anterior of the stomach. HP(+) ulccrs made up 73. 8 % of total cases. Conclusion: Peptic ulcer is a common disease in Zhejiang Province. Its incidence within one year is about 0. 084 %. The peak age is 30 - 49 years old. More male patients are ill with peptic ulcer than female ones. The highest incidence is found in administrators. The main reason of more male patients ill with peptic ulcer is that males smoke much more than females in Zhejiang. The rate of HP infection in peptic ulcer cases is much lower than that years ago.
