摘要 目的:观察行为干预加二甲双胍对代谢综合征生物学和血生化指标的影响。方法:选择门诊初治代谢综合征患者30例,随机分为单纯非药物疗法观察组(非药物组)14例和非药物疗法加二甲双胍治疗组(二甲双胍组)16例。非药物组治疗采用改善饮食及生活方式的方法,二甲双胍组除上述非药物治疗方法外,口服二甲双胍(美迪康,齐鲁制药),连续服药观察30d。观察血压、腹围,抽取空腹静脉血测定空腹血糖、血三酰甘油、高密度脂蛋白胆固醇、血浆胰岛素、胰岛素敏感性指数犤ISI=-ln(空腹血糖×空腹胰岛素)犦等指标。结果:代谢综合征患者胰岛素敏感性指数明显低于健康对照组(-4.59±0.26和-3.97±0.46;t=5.524,P<0.01),非药物组和二甲双胍组两组间治疗前各项指标无显著性差异(P>0.05)。而治疗后两组间比较二甲双胍组收缩压、舒张压、三酰甘油、空腹血糖、胰岛素均显著低于非药物组(P<0.05),胰岛素敏感性指数则显著高于非药物治疗组(P<0.05)。结论:以行为干预加二甲双胍治疗代谢综合征可改善多种代谢异常,对心血管疾病危险因素有预防作用。 AIM:To observe the effect of behavior intervention and metformin on biological and biochemical indexes in metabolic syndrome(MS). METHODS:Thirty patients with initial diagnosis of MS,were divided into non drug treated group(n=14) and metformin treated group(n=16).The non drug treated group received non drug method of improving diet and life style,and the metformin treated group received non drug method and metformin orally(Meidikang,Qilu Pharmaceutical Factory) for 30 days successively.The changes of blood pressure,abdomen circumference were observed,fasting abdominal venous blood were drawn to determine fasting blood glucose, triacylglycerol(TG),high density lipoprotein cholesterol(HDL C),plasma insulin and insulin sensitive index[(ISI),ISI=fasting blood glucose×fasting insulin]. RESULTS:The ISI of the MS patients was obviously lower than that of the healthy controls(-4.59±0.26 and -3.97±0.46,t=5.524,P< 0.01),and there were insignificant differences in the indexes before treatment between the non drug treated group and the metformin treated group(P >0.05).After treatment,the systolic pressure,diastolic pressure,TG,fasting blood glucose and insulin in the metformin treated group were significantly lower than those in the non drug treated group(P< 0.05),while the ISI was markedly higher than that in the non drug treated group(P< 0.05). CONCLUSION:Behavior intervention combined with metformin in treating metabolic syndrome improves many metabolic disorders,and has a prophylactic effect on the risk factors of cardiovascular disease.
