右美托咪定对高血压患者喉罩置入反应的影响 被引量:2

Effect of Dexmedetomidine on Laryngeal Mask Insertion Reaction in Hypertensive Patients

摘要 [目的]探索应用右美托咪定(DEX)减轻高血压患者喉罩置入反应的可行性.[方法]选择择期手术高血压患者20例,随机分为A组(对照组,n=10)和B组(DEX组,n=10).记录入室后(T0)、诱导前(T1)、喉罩置入后1 min(T2)、3min(T3)四个时间点的平均动脉压(MAP)和心率(HR),计算各时间点的心率收缩压乘积(RPP),检测T1和T3时血浆肾上腺素(E)、去甲肾上腺素(NE)、皮质醇(Cor)浓度.[结果]A组MAP、HR、RPP等指标T2时高于T1,T3 RPP高于T1(P<0.05);B组T0时MAP、HR、RPP高于T1、T2和T3(P<0.05),T1、T2、T3 3个时间点其差异无统计学意义(P>0.05);B组MAP、HR、RPP在T1、T2时低于A组(P<0.05);喉罩置入后,A组E、NE、Cor上升(P<0.05),B组没有变化(P>0.05),B组四个时间点E、NE、Cor均低于A组(P<0.05).[结论]右美托咪定能够有效抑制高血压患者全麻诱导期喉罩置入时应激反应,抑制儿茶酚胺和Cor的过度释放,维持血流动力学稳定.
