摘要
目的运用多普勒超声诊断仪检测黄芪注射液对甲状腺功能亢进性心脏病(hyperthyroid cardial disease,HCD)患者的临床治疗效果,以及心脏结构和功能变化。方法将120例HCD患者随机平均分为治疗组和对照组;对照组给予丙硫氧嘧啶、美托洛尔常规治疗,治疗组在对照组的基础上给以黄芪注射液治疗;14 d治疗后多普勒超声诊断仪检测2组患者的治疗情况;对2组患者的室间隔(interventricular septum diameter,IVSd)和左心室后壁的舒张期厚度(left ventricular posterior wall diameter,LVPWd)、右心室舒张期前后径(right ventricle diameter,RVd)、左心房收缩后期前后径(left atrial diameter,LA)、主动脉瓣根处收缩期内径(aorta,AOd)以及右心房收缩后期横径(right atrial diameter,RA)、肺主动脉收缩期内径(main pulmonary artery,MPA)进行检测计算。结果对照组患者总有效率为61.67%,治疗组总有效率达86.67%,存在显著性差异(P<0.05);治疗组患者的IVSd、LVPWd、RV、LA、AO厚度均有所下降,对照组情况无显著变化(P<0.05);治疗组的RA、MPA较对照组改善显著,差异有统计学意义(P<0.05)。结论黄芪注射液联合美托洛尔对HCD患者的治疗效果较为显著,患者的心脏结构和功能均有所改善,可用于临床治疗。
Objective To observe the clinical effect of astragalus membranaceus injection on cardiac structure and function on the patients with hyperthyroid cardiac disease( HCD) by using doppler ultrasound. Methods 120 cases with hyperthyroidism complicating hyperthyroid heart disease were randomly divided into two groups including the treatment group and control group. The control group( n = 60) : conventional propyhhiouracil plus metoprolol; treatment group( n = 60) : adding adding astragalus membranaceus injection on the basis of conventional therapy,for 14 d as a course of treatment; the indexes of interventricular septum diameter( IVSd),left ventricular posterior wall diameter( LVPWd),right ventricle( RV),left atrial diameter( LA),aorta( AO),right atrial diameter( RA) and main pulmonary artery( MPA) were observed. Results The total effective rate of treatment group( 86. 67%) wae significantly higher than that of the control group( 61. 67%). The indexes of IVSd,LVPWd,RV,LA and AO in treatment group before and after treatment were significantly diferent( P < 0. 05); while in the control group there were no significant difference. RA and MPA in the treatment group compared with the control group,the diferences were significantly different( P < 0. 05). Conclusion The curative effect of astragalus membranaceus combined metoprolol were predominent in patients with hyperthyroidism complicating hyperthyroid heart disease. It could significantly improve the cardiac structure and function,so it could be wildly applied in the clinical practice.
