摘要
目的探讨直肠全系膜切除术在低位直肠癌的作用以及该手术的关键要点.方法回顾分析自1999年1月～2002年2月进行直肠全系膜切除术的低位直肠癌根治手术102例.结果无手术死亡,系膜标本中19例检测癌结节,占17 7%;34例检测到淋巴结,占33.3%.术中吻合口漏4例,发生率为3.9%,术后吻合口瘘7例,发生率为6.9%;术后2年内肿瘤局部复发10例,复发率为9.8%.结论全直肠系膜切除能有效预防和降低直肠癌术后的局部复发率.
Objective: To investigate the curative effect of total mesorectal excision(TME) in the low rectal cancer and the key points of the operation. Methods: The clinical data of l02 cases with low rectal cancer treated by TME from January 1999 to February 2002 were retrospectively analyzed. Results: There was no operative death. In the mesorectal sample, cancer nodes was detected in 19 (17.7%) cases, lymph nodes was detected in 34 (33.3%) cases. The anastomotic leakage during operation occurred in 4 cases (3.9%). The anastomotic fistula after operation occurred in 7 patients (6.9%). The rate of local recurrence in 2 years is 9.8% (10 cases). Conclusions: TME can effectively reduce the local recurrence rate of low rectal carcinoma.
