摘要
目的 探讨高压氧 (HBO)对药物中毒耳蜗微循环的影响。方法 实验组 54只豚鼠腹腔注射庆大霉素 1 50 m g· kg- 1 · d- 1 ,连续注射 5d,造成耳聋。第 6天将豚鼠随机分为耳聋 (A)组、耳聋 +高压空气 (B)组 ,耳聋 + HBO(C)组 ,每组 1 8只动物 ,再按不同暴露时间各分成 3个小组。B组和 C组分别给予高压空气和 HBO 0 .2 MPa暴露 ,每天 1次。正常对照组 6只豚鼠不作任何处理。于药物注射前及耳聋后 1 ,1 0 ,2 0 d分别以听觉脑干诱发电位 (ABR)仪检测耳蜗听功能 ,以激光多普勒血流仪 (L DF)检测耳蜗血流量 (CBF) ,处死动物后行耳蜗毛细胞扫描电镜观察。结果 实验组较对照组动物 ABR阈移及CBF均显著降低 (P<0 .0 1 ) ,毛细胞形态明显受损。其中 C组较 A和 B组的 CBF增高 ,差异有非常显著性 (P<0 .0 1 )。结论 HBO可以显著提高豚鼠庆大霉素中毒耳蜗的血流量 ,改善了耳蜗的微循环 。
Objective To investigate the effect of hyperbaric oxygen on drug-induced,changes in cochlear microcirculation in guinea pig,and to explore its clinical significance.Methods Guinea pigs were randomly selected into two groups:the testing group(n=54)and the control group(n=6).Each of the testing ones was injected intraperitoneally with gentamicin(150mg·kg -1 ·d -1 )for five consecutive days.On the sixth day,the guinea pigs in the testing group were further divided into three sub-groups,and exposed to common-baric air(CBA group,n=18),hyperbaric air(HBA group,n=18)and hyperbaric oxygen(HBO group,n=18).The guinea pigs in the HBA and HBO groups were exposed to air and oxygen at200kPa for60minutes once a day,respectively.Auditive brain response(ABR)was measured before the administration of gentamicin,and both cochlear blood flow(CBF)and ABR were measured after the first, the tenth and the twentieth exposure.Subsequently,histological changes in the cochlear hair cells were observed by scanning electron-microscopy.For comparison,the guinea pigs in the control group did not under go any treatment.Results ABR threshold shift and CBF in the testing group were significantly reduced(P<0.01),compared with those in the control group.Histological deformation of the cochlear hair cells was observed.CBF in the HBO group was significantly increased,compared with that in HBA and CBA groups(P<0.01).Conclusions Being exposed to HBO can increase blood flow of microcirculation in gentamicin-induced toxic cochlear in guinea pigs.It is suggested that HBO might be adopted for the prevention and treatment of toxic deafness.
