摘要
目的:评价海藻酸钠-聚赖氨酸-海藻酸钠(APA)微囊化牛肾上腺嗜铬细胞(BCC)移植于患者蛛网膜下腔后的镇痛效果。方法:分4个剂量组:微胶囊0.5×107,1.0×107,1.25×107,1.5×107/(次·例)。采用腰椎穿刺术,将生理盐水稀释后的微胶囊注入20例中、重度癌痛患者L3~4或L4~5蛛网膜下腔内。结果:移植后第1~5天开始起效,移植前疼痛强度(数字分级法)为7.8±1.5,移植后降至2.5±2.3(P<0.05)。镇痛总有效率在0.5×107,1.0×107,1.25×107,1.5×107/(次·例)剂量组分别为:66%,100%,80%,66%。镇痛持续中位时间24d,最长持续时间>120d。移植后患者生活质量评分较移植前升高10分(P>0.05)。结论:APA-BCCs微胶囊蛛网膜下腔移植治疗中、重度癌痛具有起效较快、长效镇痛的特点。
AIM:To evaluate the analgesic effect of Alginate Polylysine Alyinate(APA)miroencapsulated bovine chromaffin cells(BCCs) xenotransplantated into spinal subarachnoid space of cancer patients. METHODS:Twenty cases were randomly divided into four groups: 0.5×107 cells per time group,1.0×107 cells per time group,1.25×107 cells per time group and 1.5×107 cells per time group.APA BCCs which were diluted by saline were slowly injected into the spinal subarachnoid space L3-4 or L4-5 of patients who suffered from moderate or severe cancer pain with lumbar puncture. RESULTS:The analgesic effect was apparent 5 days after xenotransplantation, and pain intensity (numerical pain intensity scale) descended from 7.8±1.5 to 2.5±2.3(P< 0.05).The total analgesic effective rates were 66%,100%,80%,66% respectively in 0.5×107,1.0×107,1.25×107,1.5×107cells per time group. The medium time of analgesia lasted 24 days, and the longest time over 120 days.The patients' quality of life improved 10 scores as compared with that of protransplantation(P >0.05). CONCLUSION:Xenotransplantation of APA BCCs into the spinal subarachnoid space against moderate and severe caner pain can be relieved promptly and stably.
