摘要
目的 血管内皮生长因子 (VEGF)在肺癌 ,特别是非小细胞肺癌 (NSCLC)生长、转移中起重要作用。本实验旨在评价外周血VEGF与肿瘤标记物细胞角蛋白 1 9(CK1 9)mRNA、癌胚抗原 (CEA)和肿瘤相关物质 (TSGF)的联合检测对NSCLC临床诊断价值。方法 采用酶联免疫法 (ELISA)等测定 1 30例肺癌、30例良性肺部疾病患者和 5 0例健康人循环VEGF、CEA和TS GF水平 ;逆转录聚合酶链反应 (RT PCR)技术检测NSCLC外周血单个核细胞CK1 9mRNA的表达情况。数据统计分析用t检验和 χ2 检验。结果 肺癌组VEGF表达水平为 (6 0 4 .4± 2 5 6 .8)pg/ml,显著高于良性肺病组和健康对照组 [分别为 (35 0 .6± 81 .2 )pg/ml和 (2 88.4± 87.0 ) pg/ml,P <0 .0 1 ]。肺癌组外周血CK1 9mRNA和血清VEGF、CEA、TSGF阳性检出率 (分别为6 0 .0 %、6 5 .4 %、6 1 .5 %和 5 5 .4 % )明显高于良性肺病组 (分别为 2 0 .0 %、36 .7%、1 6 .7%和 2 3.3% )和健康对照组 (分别为 6 .0 %、1 4 .0 %、1 0 .0 %和 1 2 .0 % ) (P <0 .0 1 )。随着临床分期逐步升高 ,CK1 9mRNA和血清VEGF阳性率及水平升高 (P <0 .0 1 )。就组织类型而言 ,VEGF阳性率及表达水平以鳞癌为高 ;CEA检测阳性率以腺癌最高 ;CK1 9mRNA和TSGF阳性检出率无统计学差别。
Objective Vascular endothelial growth factor(VEGF) is considered to play an important role in metastasis and growth of lung cancer,especially in non small cell lung cancer(NSCLC).The aim of the study is to evaluate whether the detection of VEGF combined with other tumor markers,such as CK19mRNA,carcinoembryanic antigen(CEA) and tumor supplied group of factor(TSGF),would improve the diagnosis of NSCLC.Method Serum VEGF,CEA and TSGF were determined by enzyme linked immunosorbent assay(ELISA) or radioimmunoassay(RIA) in 130 patients with NSCLC,30 with benign lung diseases(BLD) and 20 healthy controls.The expression of CK19mRNA in peripheral blood mononuclear cells(PBMCs) was analysed by RT PCR.The data were analysed by t test or χ 2 test.Results The level of VEGF in NSCLC group(604.4±256.8pg/ml) was remarkably higher than in BLD group and in healthy control group(350.6±81.2pg/ml and 288.4±87.0pg/ml,respectively)(P<0.01).The positive rate of CK19mRNA,VEGF,CEA and TSGF in NSCLC group(60.0%,65.4%,61.5% and 55.4%,respectively) were remarkably higher than that in BLD group(20.0%,36.7%,16.7% and 23.3%,respectively)and in healthy control group(6.0%,14.0%, 10.0 % and 12.0%,respectively)( P <0.01).Positive correlations were found among the circulating level of VEGF,CK19mRNA expression,and clinical stage( P <0.01).As for histological feature,VEGF and CEA were found to be relatively specific for squamous cell carcinoma and adenocarcinoma,respectively.Compared with the diagnosis based on detection of single or two tumor markers,combined detection of above three or four tumor markers yielded a significant higher accuracy in diagnosis of NSCLC( P < 0.05 ).Conclusion Combined detection of VEGF,CK19mRNA,CEA and TSGF is helpful to diagnosis and prognosis evaluation of NSCLC.
