摘要
目的 观察小儿心内直视手术围术期血清心肌酶变化特点。方法 2 0例ASAⅠ -Ⅱ级先天性心脏病房、室间隔缺损患者 ,根据年龄分为小儿组和成人组。小儿组 (Ⅰ组 ) 10例 ,年龄 2 - 12岁 ,平均 (6 .73± 2 .76 )岁 ;成人组 (Ⅱ组 ) 10例 ,年龄 17- 4 4岁 ,平均 (2 6 .4 0± 12 .0 1)岁。测定麻醉前 10分钟、转流前 10分钟、主动脉开放后30分钟、术毕 2 4小时、术毕 72小时的心肌酶浓度。结果 两组患者心肌酶在主动脉开放后 30分钟、术毕 2 4小时 ,较麻醉前均有显著性升高 (P <0 .0 1) ;而在术毕 72小时 ,小儿组除LDH外其余各项指标均已恢复到麻醉前水平(P >0 .0 5 ) ,成人组除CK -MB外其余各项指标仍保持在较高水平 (P <0 .0 1)。结论 心内直视手术后小儿心肌创伤修复较成人为快。
Objective To observe the perioperational changes of the values of serum myocardial enzymes for children undergoing open heart surgery.Methods 20 congential heart disease suffering patients,ASA physical status 1-2,were divided into 2 groups ,10 cases in children group(group1) ageing from 2-12 years old(6.73±2.76),10 cases in adult group (group 2),17-44 years old (26.4±12.01).For every case,the arterial blood samples were taken to measure the value of serum myocardial enzymes 10 minutes before anesthesia,10 minutes before CPB,30 minutes after declamping,24 and 72h postoperation.Results In all groups,the myocardial enzymes in 30 minutes after declamping,24h postoperation were increased significantly as compared with those preanesthesia .As to those of 72h postoperation ,the indexes of myocardial enzymes had arrived to the preanesthesia level except LDH in children group ,while in the adult group,the indexes of myocardial enzymes still maintain the high level except CK-MB.Conclusion After heart operation ,the cardiac muscle can heal faster in children as compared to adult.
