摘要
为从宏观上应用工程方法研究房水运行与青光眼发病机理和治疗的关系,本文提出了人眼前节房水流动系统的动力学仿真模型。本模型依据目前眼科界较为公认的房水运行和眼压反馈机制建立系统构架,基于对眼科解剖学与生理学相关理论的分析、流体力学基本原理和临床参数进行系统辨识与参数确定。对系统各项参数与原发性开角型青光眼发病机理之间的关系进行了初步的分析,分别对原发性开角型青光眼的病理状态和两种常用药物对其治疗的原理进行了仿真模拟,获得了和临床结论较为一致的结果。同时能获得一些不易测量的参数,可为模拟原发性开角型青光眼的病况和预报治疗效果提供依据。
In order to research the mechanism of the aqueous humor circulation and its relationship to the primary open angle glaucoma (POAG) macroscopically with engineering method, a dynamistic simulation model was built for the system of human eye’s aqueous humor circulation. The framework of the model was built based on the ophthalmic accepted feedback mechanism of the aqueous humor flow and intraocular pressure (IOP). The transfer functions and the parameters were educed from the analysis of physiological theories, the basal elements of hydrodynamics, and the clinical parameters. The relation between the parameters of the system and the episode mechanism of POAG was discussed. A digital method was used to simulate the Challenge test and some medicines' treatment of POAG, and the results were consistent with clinical observations. Additionally, a few parameters which are hardly captured with clinical method could be obtained from the model. These parameters can be helpful for the diagnosis and prediction of the curative effect.
