摘要
目的 探讨成年人眼肌型重症肌无力 (O -MG)的胸腺影像学变化特点。方法 对正在接受临床治疗的成年人O -MG患者 5 0例 ,男 1 8例 ,女 32例 ,年龄 2 0～ 5 5岁 ,进行磁共振成像 (MRI)检查 ,其中 1 6例增强扫描。对照组5 0例为相同年龄段非重症肌无力患者 (N -MG) ,其中 2 0例增强扫描。观察两组胸腺变化关系包括脂肪变、增生、占位效应等。结果 (1 )胸腺脂肪变O -MG组 2 5例 ,对照组 8例。 (2 )胸腺增大 :O -MG组 4例 ,其中 2例含结节灶 ;对照组无一例增大。 (3)胸腺出现占位病灶仅见于O -MG组 5例。 (4)胸腺未发现异常 :O -MG组 1 6例 ,对照组 4 2例。 (5 )统计学处理显示 :胸腺脂肪变O -MG组与对照组比较差异有极显著性 (P <0 .0 0 1 ) ;胸腺的异常率比较 ,两组同样具有极显著性差异 (P <0 .0 0 1 )。结论 胸腺脂肪变是成年人O -MG患者常见和主要异常变化。MRI能提供胸腺形态和组织变化的客观信息 ,有助于临床治疗方式的选择和疗效观察。
Objective To study MRI findings of the thymus gland in adult patients with ocular-muscular type myasthenia gravis (O-MG). Methods 50 cases adult patients with O-MG (male : female = 18 : 32) were performed MRI scanning and put the stress on the morphological and signal changes of thymus.Results The thymus gland had been partly replaced by fattiness tissue in 25 cases (50%), and enlarged thymus shown in 4 cases in which non-nodular enlargement revealed in 2 cases and with nodular enlargement in 2 cases. Only 5 cases had occupy effect seen in thymus. Conclusion Thymus gland replaced by fat molecule tissue is a usually and mainly sign in adult patients due to O-MG, for more further to understand the thymus gland in adult patients with O-MG, MRI examination should be useful.
