肥胖的2型糖尿病患者代谢状态的临床分析 被引量:9

摘要 目的 通过观察肥胖和非肥胖的 2型糖尿病患者的代谢状态 ,了解肥胖的 2型糖尿病患者代谢紊乱的特点。方法 将 88例2型糖尿病患者按体重指数 (BMI)分为 2组 ,肥胖组 48例 (BMI≥ 2 5kg/m2 )和非肥胖组 40例 (BMI <2 5kg/m2 )。测定血压 (SBP/DBP)、空腹血糖 (FPG)、糖化血红蛋白 (HbA1C)、血浆空腹胰岛素 (F -ins)、血总胆固醇 (Tch)、甘油三酯 (TG)、高密度脂蛋白胆固醇 (HDL -c)、低密度脂蛋白胆固醇 (LDL -c)、2 4小时尿白蛋白排泄率 (UAER)、血尿酸 (BUA)。结果  2型糖尿病肥胖组SBP ,TG ,LDL -c ,F -ins ,UAER ,BUA明显高于非肥胖组 ,HDL -c较非肥胖组明显低 ,两组间比较均有统计学意义 ;Tch ,FPG ,HbA1C水平两组之间无明显差异。结论 肥胖的 2型糖尿病患者有明显的代谢紊乱。BMI是影响糖尿病患者脂代谢、血压、胰岛素水平、血尿酸水平及尿白蛋白排泄率的重要因素。对 2型糖尿病伴有肥胖的患者应予以重视 。 Objective To investigate characteristics of the metabolic state in type 2 diabetes patients with obesity for prevention and management of diabetes complication, we have observed 48 obese type 2 diabetes patients and 40 nonobese patients in this study. Methods 88 cases of type 2 diabetes patients were divided into two groups by BMI, in which there are 48 obese patients and 40 nonobese patients. The levels of blood pressure (SBP/DBP), fasting plasma glucose (FPG), HbA1C, plasma insulin (F-ins), blood lipid ( Tch, TG, LDL-c, HDL-c), blood uric acid (BUA) and urine albumin excretion rate of 24 hours (UAER) were detected. Results Systolic blood pressure (SBP) level was significantly higher in type 2 diabetes with obesity than those in control . Plasma insulin level was significantly higher in type 2 diabetes with obesity than those in non-obese group. TG and LDL-c levels significantly increased, and HDL-c level significantly decreased in obesity group. Urine albumin excretion rate of 24 hours and blood uric acid significantly increased in obesity group. Conclusion Obesity type 2 diabetes patients suffer from a metabolic disorder, thereby, type 2 diabetes patients with obesity must attach great importance for effectively controlling the metabolic disorder and preventing the development of complications .
