体外循环心脏直视手术后肾功能衰竭危险因素分析 被引量:11

Analysis of Correlations between Low-perfusion and Renal Failure after Open-heart Surgery with Cardio-pulmonary Bypass

摘要 目的 探讨分析体外循环术后肾功能衰竭的相关危险因素。方法 根据术后有无肾功能异常 ,将 6 0例体外循环心脏直视手术病人分为两组 ,对比分析了体外循环诸多因素与肾功能不全发生的相关性。结果 肾功能异常组在体外循环时间、低灌注发生率、升压药用量、输血量和血流动力学稳定性方面与正常组对照有显著差别。结论 心功能不全、体外循环时间长、围术期低血压、大量应用升压药和血制品以及术前高血压。 OBJECTIVE To explore and analyse the risk factors which lead to renal failure after cardiopulmonary bypass surgery.METHODS 60 patients were divided into two groups based on whether they have renal failure after the open-heart surgerys. Correlated analysis was conducted to demonstrate the correlations between various factors and renal failure. RESULTS There are significant difference between two groups in preoperative heart function, cardiopulmonary bypass time, occurrence frequency of lower peri-operative blood pressure, high dosage of catecholamine and bank blood.CONCLUSION Poor preoperative heart function, longer cardiopulmonary bypass time, lower peri-operative blood pressure, huge dosage of catecholamine and bank blood were believed to be important factors for the postoperative renal dysfunction. Preoperative hypertension and diabetes were also considered as risk factors for renal failure.
