摘要
影像医学正在从传统的单纯形态学检查向着形态学和反映细胞、分子水平生理生化改变的功能学检查相结合的方向发展。本文重点讨论了CT灌注成像、MR扩散加权成像、MR灌注成像的基本原理以及在肝脏病变,特别是肝硬化和肝脏肿瘤诊断中的应用。肝硬化病人灌注成像显示HAP增加,而PVP下降;肝脏恶性肿瘤病人HAF明显增高,磁共振DWI及ADC值的测量可以鉴别肝脏囊性良性病变与实质性恶性病变。文章同时探讨了MR波谱及特异性对比剂在肝脏病变诊断中的作用。CT、MR功能成像与形态学检查的结合有助于提高病变的诊断和鉴别诊断。
Medical imaging are developing from the pattern of traditional morphologic examination to the comprehensive pattern that combine morphology with functional examination.This article emphatically discuss the basic priciples of CT perfusion imaging,MR diffusion weighted imaging,MR perfusion imagˉing and MR spectroscopy,and the application in diagnosis of liver diseases,especially cirrhosis and hepatic tumor.Perfusion imaging showes HAP increased insteased of PVP decreased in the cirrhosis patient,and HAF distinct increased in the hepatoma patients.MR DWI and ADC value can be use to difˉferentiate benign liver cystic lesion from malignant solid tumor.This article also discuss the application of MR spectroscopy and liver-specific congtrast medium in diagnosis of liver diseases.CT&MR funcˉtional imaging with morphologic examination can be conducive to diagnosis and differential diagnosis.
