摘要 背景:足底压力测量可以获取人体在各体态下的生理和病理力学参数,已经成为诊断疾病和评价康复的重要手段。目的:研制一套足底压力分布测量系统并评价其临床应用价值。设计:足底压力测量系统设计及病例对照研究。地点和对象:在华中科技大学同济医院完成。门诊和住院糖尿病患者共145例和正常人80例。干预:系统硬件包括一个由200个国产微型力传感器组成的300mm×150mm的传感器矩阵、接口电路、信号采集及计算机系统;系统软件主要由实时数据采集模块、数据存贮模块、数据计算分析和显示模块、病历管理与资料查询模块、诊断模块等组成。应用本系统对80例正常人(正常组)、102例无明显周围神经病变的糖尿病患者(D组)及M43例合并周围神经病变的糖尿病患者(DN组)进行足底压力测试。主要观察指标:各组动态足底各部位最大压力和足底峰值压力。结果:DN组足底最大压力为(4.037±0.999)kg/cm2,比正常组犤(2.886±0.562)kg/cm2犦明显增高,差异有显著性意义(P<0.001);而DM组足底最大压力为(3.004±0.708)kg/cm,与正常组相似,差异2无显著性意义(P>0.05)。结论:足底压力分布测量系统稳定、灵敏度和分辨率高、结果可靠、操作简便。 BACKGROUND:Plantar pressure measurement may provide the physiological and pathological parameters in all kinds of posture,and has become an important method in the diagnosis of disease and the assessment of rehabilitation. OBJECTIVE:To develop a measuring system for plantar pressure and evaluate the value of its clinical application. DESIGN:Design of measuring system for plantar pressure and a case controlled trial. SETTING and PARTICIPANTS:The study was conducted in the Tongji Hospital of Huazhong University of Science and Technology.A total of 145 diabetic patients selected from both outpatient and inpatient departments and 80 normal individuals were included in our study. INTERVENTIONS:The system was composed of hardware part and software part.The hardware part includes sensor matrix containing 200 domestic micro pressure sensors in a size of 300 mm×150 mm,interface circuit,signal collector,and computer system.The software part consists of modules for real time data collection,storage,analysis,display,diagnosis,case management and inquiry.Plantar pressure was measured by the system in 80 normal individuals(control group) and 145 diabetic patients including 43 patients with peripheral neuropathy(DN group) and 102 patients without distinct peripheral neuropathy(DM group). RESULTS:The average peak plantar pressure in DN group[(4.037 ±0.999) kg/cm2] was significantly higher than that of DM group[(3.00 4 ±0.708 0) kg/cm2] and that of control group[(2.886±0.562) kg/cm2](P< 0.001),but no significant difference was found between DM group and control group(P >0.05). CONCLUSION:The measuring system for plantar pressure distribution has high stabilization,sensitivity and differentiability,and can be operated reliably and simply.
