摘要
目的 探讨完全性肺静脉异位引流 (TAPVD)的手术方法及效果。方法 2 2例TAPVD患者 ,其中心上型 16例 ,采用心房横断 (14例 )和心上吻合 (2例 ) ,行左房 共同静脉吻合 ;心内型 4例 ,将房间隔缺损扩大 ,应用补片将异位引流的肺静脉隔至左房 ;混合型 2例 ,上腔静脉用心包补片加宽。结果 2 0例痊愈 ,2例死亡。痊愈患者均获得随访 ,随访时间 8个月～ 6年 ,心功能均正常。结论 TAPVD患者一旦诊断明确 ,应及早手术 ,选择适当的手术方法可获良好的治疗效果。
Objective To summarize the surgical treatment and its effect on total anomonous pulmonary venous drainage(TAPVD).Methods Twenty-two patients were included in the study.Sixteen patients were the supracardiac,four patients the intracardiac and tow the mixed type. In supracardica group, fourteen underwent transatrial method and tow with supracardiac method. Patients of intracardiac type underwent patch closure of the atrial septal defect.A pericardial patch was used to widen the superior vena cava in the mixed type patients. Results Twenty patients had uneventful postoperative recovery, and two died. Conclusion Early diagnosis and operate are very important in TAPVD patients.An appropriate choice of surgical method can ensure a satisfactory outcome.
