260株念珠菌分离鉴定及耐药性分析 被引量:49

摘要 目的 探讨引起医院感染常见酵母样真菌种类及其耐药性特点 ,为临床感染性疾病提供病原学诊断和合理使用抗真菌药物的依据。方法 采集 2 6 80份住院患者标本分离培养 ,分离出酵母样真菌 2 6 0株 ,采用科码嘉念珠菌显色培养基 (CHRO- Magar)培养 ,API- 2 0 CAUX酵母样真菌鉴定试条进行鉴定 ;念珠菌药敏试条 (ATB-Fungus)进行念珠菌药敏试验。结果  2 6 0株酵母样真菌以念珠菌为主 ,其中白色念珠菌 15 5株 (5 9.6 % ) ,热带念珠菌 5 1株 (19.6 % ) ,克柔念珠菌 2 7株 (10 .3% ) ,光滑念珠菌 11株 (4 .2 % ) ;白色念珠菌对两性霉素 B(AMB)、5 -氟胞嘧啶 (5 - FC)、制霉菌素 (NYS)、酮康唑 (KTC)、益康唑 (ECO)、咪康唑 (MTC)、氟康唑 (FL C)的耐药率分别为9.0 %、5 .8%、10 .3%、4 3.0 %、4 0 .9%、4 3.7%、9.0 % ;热带念珠菌分别为 3.9%、9.8%、19.0 %、6 1.9%、5 7.1%、6 1.9%、2 1.4 % ;克柔念珠菌为 14 .8%、14 .8%、14 .8%、14 .8%、2 5 .0 %、2 5 .0 %、16 .6 % ;光滑念珠菌为 0 .0、0 .0、36 .6 %、18.1%、36 .6 %、36 .3%、6 6 .6 %。结论 目前引起我院患者真菌感染的念珠菌仍以白色念珠菌为主 ,其次是热带念珠菌及克柔念珠菌 ,这些念珠菌对两性霉素 B、5 -氟胞嘧啶、氟康唑、制霉菌素较敏感 ,? OBJECTIVE To investigate the kinds of yeast-like fungi causing hospital infection and drug resistance characteristics of antibiotics, in order to provide guideline of etiologic diagnosis and suitable anti-fungus medication for clinical infectious diseases. METHODS A total of 2 680 samples of inpatients were cultured and 260 strains of yeast-like fungi were identified with CHRO-Magar medium and API-20 CAUX strip for yeast-like fungi. Drug sensitive test of Candida was detected with ATB-Fungus trip. RESULTS Form them Candida albicans accounted for 59.6% (155 strains), C.tropicalis 19.6% (51 strains), C.krusei 10.3% (27 strains), and C.glabrata 4.2% (11 strains). The sensitive rates of the four kinds of Candida to amphotericine B (AMB), flucytosine (5-FC), nystatin (NYS), ketoconazole (KTC), econazole (ECO), miconazole (MIC), and fluconazole (FLC) were from 0.0 to 66.6%. CONCLUSIONS The pathogens causing fungus infection in our hospital are mainly C.albicans. Their resistance characteristics to AMB, 5-FC, FLC and NYS are relatively weaker, but to KTC, ECO, and MIC are stronger.
