The surgical treatment of the pelvis fractures

摘要 目的 对骨盆骨折 (骨盆环及髋臼 )手术治疗结果进行回顾性分析 ,总结最佳治疗方案。方法  86例骨盆骨折中骨盆环骨折 32例 ,髋臼骨折 5 4例。骨盆环骨折按改良Tile分型 ,髋臼骨折按letournel分型。对其行切开复位螺丝钉和重建钢板固定或闭合复位外固定架固定。结果 平均随访时间为 31.2月 (4- 34月 ) ,根据d’Aubigne和Postel评分系统 :骨盆环骨折 32例中 ,优 15例 (46 .9% ) ,良 7例 (2 1.8% ) ,一般 6例 (18.8% ) ,差 4例(12 .5 % ) ,优良率 6 8.7%。髋臼骨折 5 4例中 ,优 18例 (33.3% ) ,良 2 8例 (5 1.9% ) ,一般 5例 (9.3% ) ,差 3例(5 .5 % ) ,优良率 85 .2 %。结论 对骨盆环和髋臼骨折正确分型 ,合适的手术入路 ,正确选择内固定物及可靠固定 ,及时手术 ,解剖复位 ,对提高疗效是非常关键的。 Objective To sum up the best ways by retrospectively analying the clinical results of surgical treatment of the pelvis (ring and acetabulum ) fractures.Methods In this cases, 86 palvic fractures(consist of 32 pelvic ring and 54 acetabular fractures) were reviewed and classified(according to the modified Tile and Letournel classification respectively). Fracture open reduction and fixation used screws and reconstruction plates and closed reduction and fixation used the external fixator.Results The average follow-up time was 31.2 months(ranging from 4-34 months). In the 32 pelvic ring fractures, 15 cases showed excellent results, 7 good, 6 fair, 4 poor, and the excellent and good rate was 68.7%. In the 54 acetabular fractures, 18 excellent, 28 good, 5 fair, 3 poor,and the excellent and good rate was 85.2, according to the d' Aubigne and Postel Score System.Conclusion Correct classification of fractures,appropriate selection of operative indication and approach and internal fixators ,timely operation, anatomical reduction are the key to acquire the good clinical results of treatment of pelvic ring and acetabular fractures.
