摘要
目的 研究脑囊虫患者是否存在血 -脑脊液屏障 ( BBB)功能障碍及其与细胞因子产生是否相关 ,以期对临床诊治有所帮助。方法 用单向免疫扩散法检测 41例脑囊虫患者 BBB功能 ;用 ELISA法检测患者血清和脑脊液 ( CSF)中 γ-干扰素 ( IFN-γ)及肿瘤坏死因子 ( TNF-α)水平 ;观察 BBB功能与 IFN-γ、TNF-α产生的关系。结果 ( 1 )脑囊虫患者中有 82 .9%出现 BBB损害。 ( 2 )脑囊虫患者组血清和 CSF中 IFN-γ水平较对照组明显降低(均 P <0 .0 1 ) ,TNF-α水平较对照组明显增高 ( P <0 .0 5 ,P <0 .0 1 )。 ( 3 ) BBB损害组 CSF中 IFN-γ水平较BBB正常组明显减低 ( P <0 .0 5 ) ,而血清中 IFN-γ无变化。 ( 4) BBB损害组 CSF中 TNF-α水平较 BBB正常组无明显增高 ( P >0 .0 5 ) ,而血清中 TNF-α水平较 BBB正常组明显增高 ( P <0 .0 5 )。结论 脑囊虫患者多出现BBB损害且与细胞因子水平有明显相关性 ,同时存在体液免疫增高和细胞免疫减低。
Objective To evaluate whether there are some disorders in blood brain barrior (BBB) function are correlated between cytokins and BBB, which may aid the diagnosis and treatment of patients with cerebral cysticercosis. Methods BBB function were detected by SIRD in 41 patients (F21,M20) with cerebral cysticercosis; The level of IFN γ and TNF α were tested by ELISA. Results (1)There was abnormal BBB in cerebral cysticercosis group (82.9%).(2)The IFN γ level of serum and CSF in cerebral cysticercosis group decreased distinctively as compared with control group (P<0 01). The TNF α level increased distinctively in both serum and CSF (P<0 05,P<0 01).(3)The IFN γ level of CSF in group of BBB damage decreased distinctively in comparison with BBB normal group (P<0 05), while that of serum were normal.(4)The TNF α level increased distinctively in serum (P<0 05) but not in CSF (P> 05). Conclusions The study suggests that there is a dysfunction of BBB and correlated with cytokins production in patients with cerebral cysticercosis and also enhancement of humoral disimmunity and poorness of cellular disimmunity.
