摘要
[目的] 探讨溃疡“完全愈合”的组织学特征。[方法] 观察大鼠乙酸胃溃疡形成后 5、30、126 d 3 个时间段再生黏膜的病理组织学变化及西咪替丁、柴胡桂枝汤(CHGZT)对其影响。[结果] ①第 5 天 CHGZT组溃疡边缘胃体腺颈部,可见 CHGZT 溃疡伴随细胞系(UACL)增殖、移行,其基底部去分化壁细胞广泛分布;第 30 天CHGZT UACL形如芽状,壁细胞罕见;第126天CHGZT UACL明显分化,呈现泌酸腺雏形。②第 30 天 CHGZT组含CHGZT UACL的标本数与模型对照组相比,差异有统计学意义(P<0.05)。③第 30 天胃体侧的 CHGZT UACL的高度与同一切面再生黏膜高度呈正相直线相关关系( r=0.778, P<0. 01);第 30 天 CHGZT组胃体侧再生黏膜高度与正常组胃体黏膜高度相比,差异无统计学意义。[结论] 壁细胞去分化增殖、再分化重构泌酸腺,是胃体溃疡“完全愈合”的组织学特征。
Objective] To probe into the histological characteristic of ulcer “total healing”. [Methods] Pathohistological changes of the regenerated mucous membrane and the influence of cimetidine and Chaihu Guizhi decoction (CGD) on the ulcer were observed on the 5th d, 30th d,126th d after the formation of gastric ulcer by acetic acid in rats. [Results] ① In CGD group, ulceration associated cell lineage (UACL) proliferated and conversed on the neck of gastric serif gland at the edge of ulcer on the 5th d, on the bottom of UACL dedifferentiated parietal cell distributed extensively; on the 30th d, UACL was similar to bud in shape and parietal cells was rare; on the 126th d, UACL dedifferentiated obviously and oxyntic gland came into an embryonic shape.②On the 30th day, the number of samples with UACL in CGD group was more than that of the model group(P< 0.05 ). ③ The height of UACL on gastric serif side in the CGD group was positively related to that of regenerated mucous membrane on the same section(r= 0.778 , P< 0.01 );On the 30 th day, there was no statistical difference between the height of regenerated mucous membrane and that of normal ones. [Conclusion] The proliferation of dedifferentiated parietal cells and the reconstruction of oxyntic gland are the histological characteristic of ulcer “total healing”.
