摘要
报告1例晚期舌癌患者实施多器官联合切除同期双游离瓣修复术,术中切除全舌、双侧口底、右下颌骨及左下颌骨颏部、部分右上颌骨、右软腭、右扁桃体、会厌、舌骨及全喉,制备右腓骨肌瓣、左腹直肌肌皮瓣移植修复软硬组织复合缺损,临床疗效满意。推荐对晚期口腔癌患者施行救治性外科及重建。
To evaluate the feasibility of salvage surgery in the management of advanced oral cancer, and to investigate the outcome of simultaneous reconstruction of large composite defects with double free flaps transfer. In June 2004, an advanced case of oral squamous cell carcinoma(T4N0M0)was treated by salvage surgery. The patient underwent composite resection of tumor including total glossectomy, resection of total floor-of-mouth, resection of right mandible, partial resection of right mandible, partial maxillectomy, partial resection of soft palate, tonsillectomy, resection of epiglottis, resection of hyoid bone, and total laryngectomy. Simultaneous reconstruction was carried out with free rectus abdominis myocutaneous flap and fibula osteocutaneous flap to reconstruct the composite defects resulting from oncologic resection. The patient recovered uneventfully from surgery, with no immediate postoperative complications.The transplanted flaps survived. The patient demonstrated satisfactory postoperative figure. The clinical results so far are inspiring. We consider that salvage surgery is a feasible method in the treatment of advanced oral cancer. The double free flaps transfer for reconstruction of extensive defects after salvage surgery is effective and can greatly improve the quality of life.
