摘要
目的观察乌司他丁对心脏不停跳冠状动脉搭桥术(OPCABG)患者围术期血浆促炎性细胞因子代谢水平的影响.方法22例行OPCABG患者随机分为两组:乌司他丁组(U组),切皮前(麻醉诱导后)将乌司他丁12 kIU·kg-1·h-1稀释至100ml生理盐水中持续静脉输注;对照组(C组),以等量生理盐水代替.分别于切皮前(T1)、切开心包时(T2)、旁路血管开放时(T3)及术毕(T4)抽取静脉血采用放免法测定血浆白细胞介素-6(IL-6)、白细胞介素-8(IL-8)及肿瘤坏死因子α(TNF-α)浓度.结果C组IL-6浓度在手术开始后T2~T4时段均较T1明显升高(P<0.05);U组IL-6浓度较对照组明显降低(P<0.05);U组IL-8和TNF-α在T2~T4时段较对照组显著降低(P<0.05).结论心脏OP-CABG术中应用乌司他丁能显著抑制IL-6、IL-8、TNF-α等促炎性细胞因子的释放.
Objective To observe the effect of ulinastatin on proinflamatory cytokine during off-pump coronary artery bypass grafting (OPCABG).Methods Twenty- two patients scheduled for selective OPCABG were randomly divided into two groups with 11 cases each. The patients in ulinastatin group(U) received ulinastatin 12kIU iv slowly after induction of anesthesia,and the patients in control group (C) the same volume of normal saline as in group U instead of ulinastatin. Blood samples were taken before skin incision (T_1),at dissecting pericadia (T_2),opening bypass vessel (T_3) and at the end of operation (T_4) for the measurements of interleukin-6(IL-6),IL-8 and tumor necrosis factor alpha(TNF-α) by radioimmunoassay.Results IL-6 increased significantly during the period of T_2-T_4,compared with that at T_1 in group C(P<0.05). In group U,the IL-6 decreased significantly at T_2,T_3 and T_4,compared with group C and at T_1 (P<0.05). The IL-8 and TNF-α were depressed significantly in group U,compared with those in group C and at T_1 (P<0.05).Conclusion Ulinastatin can effectively depress the proinflamatory cytokine IL-6,IL-8 and TNF-α during off-pump coronary artery bypass grafting.
