摘要
目的 比较三种AO颈椎前路钢板系统在颈前路固定中的应用特点及效果。方法 自 1998～ 2 0 0 3年 ,应用三种不同AO钢板手术治疗颈椎外伤及颈椎病共 112例 ,按所使用内固定方式不同分为标准CSLP组、小型 (亚洲人专用 )CSLP组及CSLP -VA组三组 ,记录各组术中螺钉及钢板放置困难发生率。术后分别经 3、 6、 12个月随访 ,观察钢板倾斜率、螺钉脱出率及骨不融合率。结果 所有病例术中钢板螺钉均安装成功 ,CSLP组术中螺钉植入困难率为 2 1% ,较CSLP -VA钢板组 (3% )大 (P<0 0 5 ) ;钢板倾斜率为 14 % ,较CSLP -VA组 (3% )大 (P <0 0 5 ) ;钢板放置困难发生率为 8% ,与CSLP -VA钢板组相当(P <0 0 5 )。结论 CSLP -VA钢板螺钉植入方便 ,但如操作不熟练 ,易致钢板倾斜 ;CSLP -VA更适合上颈椎损伤 ,小型CSLP则更适合于单节段固定者。
Objective To comparer the different characteristics and clinical outcomes of three types of AO spine locking plate system in anterior cervical decompression and fixation.Methods From 1998 to 2003,112 cases of cervical spine injury and cervical spondylosis were treated surgically with AO self-lock plate.They were classified as CSLP group,Asian CSLP group and CSLP-VA group according to the different plate used.The difficulty rate of screw insertion and plate implantation was recorded in operation, and plate location and bone arthrodesis during follow-up were assessed by X-ray.Results Fixation finally succeed in all patients,but difficulty rate of screw insertion in CSLP group(21%) was greater comparing with the CSLP-VA group(3%)(P<0.05);the rate of oblique location of plate in CSLP-VA group (14%) was higher than in CSLP group(3%)(P<0.05);the difficulty rate of plate implantation in CSLP-VA group and CSLP group had no significant difference(P>0.05).Conclusion CSLP-VA has the advantage of easy insertion but also tending to be oblique location in the cervical spine CSLP-VA,it is more suitable for the upper cervical spine and the Asian CSLP is more suitable for single level fixation.
