摘要
目的:探讨产科因素致新生儿缺氧缺血性脑病(HIE)原因,采取预防措施,降低HIE发病率,减少新生儿死亡及伤残.方法:回顾性分析1997年1月～2002年12月发生182例HIE,采取预防措施前后3年比较发病率、窒息率及转归.结果:182例中产科前3位相关因素分别为脐带因素(26.37%)、重度妊高征(17.03%)、早产(15.93%);前3年新生儿HIE发病率为5.38%,后3年为3.50%,前后比较差异有显著性(x2=8.85,P＜0.01);前3年窒息占54.60%,后3年占45.39%,前后比较差异有显著性(x2=6.87,P＜0.01);前3年康复、致残、死亡发生率与后3年比较差异有显著性(x2=5.97,P＜0.01).结论:重视产前检查,提高产科质量,对HIE患儿及时随访,开展早期康复治疗,是降低HIE发病率,减少死亡及伤残的重要措施.
Objective:By exploring the obstetrical factors resulting in neonates' hypoxic ischemic encephalopathy (HIE), to take preventive measurements to reduce the prevalence of HIE and neonates' death and deformity Methods:The prevalence, asphyxia rate and outcome of 182 neonates with HIE before and after adopted preventive measurements, from Jan 1997 to Dec 2002, were retrospectively analyzed Results:The former three correlated obstetrical factors were umbilical cord (26 37%), pregnancy induced hypertension (17 03%) and premature labor (15 93%). There was significant difference between the prevalence of neonates' HIE in former three years (5 38%) and that in latter three years (3 50%)( χ 2 =8 85, P <0 01). There was significant difference between the asphyxia rate in the former three years (54 60%) and that in the latter three years (45 39%)( χ 2 =6 87, P <0 01) There was significant difference in recovery, deformity and death rate between the former three years and the latter three years ( χ 2 =5 97, P <0 01) Conclusion:Paying more attention to antenatal medical examination, promoting obstetrical quality, following up timely the neonates with HIE and developing early recovery treatment should be the important measurements for reducing the prevalence of HIE, neonates' death and deformity
