运动疗法在膝关节置换术后康复治疗中的应用 被引量:4

摘要 目的:文章将近年来有关患者在膝关节置换术后应用运动疗法促进康复,加速功能恢复的研究作一综述,为临床医生指导患者应用提供科学依据。资料来源:应用计算机检索Medline1980-01/2004-09和Embase1980-10/2004-09期间的相关文章,检索词Arthroplasty,permuta-tion,knee,cinesiateics,rehabilitation,并限定文章语言种类为English。同时计算机检索www.zglckf.com2000-01/2004-04期间的相关文章,限定文章语言种类为中文,检索词“关节成形术,置换,膝;运动疗法;康复。资料选择:对资料进行初审,选取试验包括临床研究、经验总结及病例报告。并查找全文。资料提炼:共收集到21篇关于运动疗法在膝关节置换术后康复中应用的文章,其中10篇为手术前康复教育和训练,11篇为手术后康复治疗。资料综合:20个试验包括4171例患者,证明力量训练能够促进神经运动的相互联系及肌肉骨骼的完整性,持续被动运动可以防止术后黏连,缩短术后恢复时间,耐力训练有助于加强心血管系统的功能。结论:膝关节置换术后,运动疗法能够提高和促进患者神经系统、心血管系统、呼吸系统、骨关节系统各种功能的恢复,最大限度地恢复患者生活自理能力和运动能力,保持良好的身心状态。 OBJECTIVE:To review the latest achievements of application of cinesiateics in promoting rehabilitation and accelerating functional restoration in patients after knee joint replacement so as to provide scientific evidences for the clinical physicians to instruct the patients to perform rehabilitative exercise.DATA SOURCES:The related articles between January 1980 and September 2004 in Medline and October 1980 and September 2004 in Embase were searched for by inputting the key words “Arthroplasty,permutation,knee,cinesiateics,rehabilitation' in English into the computer.Similarly,the relevant articles between January 2000 and April 2004 were searched for by browsing www.zglckf.com with the key words as same as above in Chinese.STUDY SELECTION:Having been looked through primarily,the articles about clinical studies,experience review and case reports were selected,and then the full texts of them were looked up.DATA EXTRACTION:A total of 21 relevant articles to cinesiateics for the rehabilitation of knee joints after knee arthroplasty were collected,including 10 articles about rehabilitative teaching and training before operation and 11 articles about postoperative rehabilitative treatment.DATA SYNTHESIS:As indicated in 20 cinesiateics trials for 4 171 patients,strength training could improve the interrelationship of nerve motions and keep the musculoskeletal integrity,continuous passive motion could prevent knee joints from postoperative adhesion and shorten the time for postoperative recovery,and endurance training was helpful to enhance the function of cardiovascular system.CONCLUSION:After knee joint replacement,cinesiateics can accelerate functional restoration of nervous,cardiovascular,respiratory and osteoarticular systems,enhance selfcare ability,work and motor capacities,as well as make patients keep better physical and mental status.
