摘要
目的探讨老年期高血压病患者心脏结构的改变与心律失常的关系。方法 10 0例老年期高血压病患者和 10 0例正常血压老年人 ,分别作超声心动图检查和 2 4h动态心电图。结果老年高血压病组中各超声指标包括室壁厚度、左房内径、左室重量、左房容积等均明显高于对照组 ;老年高血压病组短阵房速、复杂性室性心律失常、总室性心律失常发生率均明显高于对照组 ;老年高血压病左室肥厚组短阵房速、复杂性室性心律失常、心房颤动发生率均极明显高于非左室肥厚组 ;与正常构型组相比较 ,向心性肥厚组、离心性肥厚组患者短阵房速、复杂性室性心律失常、总室性心律失常发生率均明显增高 ,向心性重构组总室性心律失常发生率明显增高。结论老年高血压病患者较正常血压老年人存在明显的室壁增厚 ,左室重量增加 ,左房增大 ,在此结构改变的基础上 ,各种心律失常包括短阵房速、房颤。
Objective To discuss the relationship between the changes of cardiac structure and arrhythmia in senile hypertensive patients. Methods 100 senile hypertensive patients as well as 100 senile citizens with normal blood pressure underwent echocardiography and dynamic electrocardiogram. Results Those indexes such as thickness of septa interventriculare, mass of left ventricle, distance of left atrium and volume of left atrium in hypertension group were remarkably greater than those in control. The incidence rates of paroxysmal superventricualr, complex ventricular arrhythmia and total ventricular arrhythmia of hypertension group were obviously higher than those of control. The incidence rates of paroxysmal superventricular, complex ventricular arrhythmia and atrial fibrillation in left ventricular hypertrophy patients increased than those without hypertrophy. The incidences rates of paroxysmal superventricualr, complex ventricular arrhythmia and total ventricular arrhythmia in concentric hypertrophy and eccentric hypertrophy patients were far more than those with normal geometry. Conclusion With the changes, such as thickend left ventricular wall, increased ventricular weight and left atrial volume in senile hypertensive patients, various arrhythmias such as paroxysmal superventricular, atrial fibrillation and complex ventricular arrhythmia occurred more frequently.
