摘要 复杂Pilon骨折的治疗最具有挑战性。要使Pilon骨折的治疗获得满意的效果首先必须对损伤进行正确的评估,其中对软组织损伤的评估对治疗计划的制定具有至关重要的作用;骨折的严重程度与预后密切相关。治疗计划包括手术时机,固定方式的选择及术后处理。本文结合文献回顾及作者的经验,提出了损伤评估的标准;对各种治疗方法进行了评价,并提出了固定方式选择的原则;同时对术后治疗提出方案。 Complex Pilon fracture is always a challenge to orthopaedic surgeons. Careful assessment of the injury, especially the soft tissue condition, is essential for a proper treatment planning. Classification of the fracture is also important for evaluation of the prognosis. Treatment planning includes: timing of the surgery, selection of the fixation method and postoperative protocols. In this article, on the basis of literature review and personal experience, the author suggests criteria for injury assessment and postoperative treatment. A brief comparison of different kinds of fixation is made and the principle of implant selection is discussed in the text.
