摘要
目的 :分析262例双向分化恶性肿瘤临床资料及免疫组织化学染色的结果。方法 :收集1950～2000年天津肿瘤医院库存双向分化恶性肿瘤组织的石蜡包埋样本约800余例 ,筛选临床和病理资料完整的样本262例 (其中恶性黑色素瘤124例 ,滑膜肉瘤64例 ,横纹肌肉瘤47例 ,间皮肉瘤23例 ,上皮样肉瘤4例 ) ,制作成组织芯片 ,进行免疫组化染色 ,分析不同的免疫组化指标在肿瘤诊断中的作用 ,并对以前的病理诊断进行复习。在此基础上分析不同肿瘤的发病年龄高峰、性别比例、发病部位及预后。结果 :124例恶性黑色素瘤有8例不表达HMB45 ,不同双向分化恶性肿瘤的发病高峰不一致 ,手术治疗后患者生存时间也不同。结论 :双向分化恶性肿瘤中免疫组化染色特异性较差 ,综合分析几种免疫组化指标有助于病理诊断。
Objective:To investigate the clinical data and immunohistochemical results of in262cases with bi-directional differential malignant tumors.Methods:262cases of bi-direcˉtional differential malignant tumors including124cases of melanoma,64cases of synovial sarcoma,47cases of rhabdomyosarcoma and mesothelial sarcoma were obtained from the Tumor Tissue Bank of Tianjin Cancer Hospital that had been accumulated from1955to2000.These samples were sectioned again and then made into tissue microarray.These samples diagnosis were confirmed after immunohistochemical staining.Based on the results of immunohistoˉchemical staining,the incidence-age-peak,ratio of male to female and prognosis were staˉtistically analyzed.Results:HMB45protein was not expressed in8cases of melanoma and1cases of malignant fibrohistoma was misdiagnosed to alveolar rhabdomyosarcomas.The inciˉdence-age-peak and prognosis are different in bi-directional differential malignant tumors.Conclusion:Single immunohistochemical staining is poorly helpful to diagnosing bi-directional differential malignant tumors.Integrated assay of several marks can contribute to the pathoˉlogical diagnosis.
