摘要
目的调查健康中国成人网织红细胞多参数分析的参考值范围,并进一步观察地中海贫血时网织红细胞各参数的改变情况及其临床意义.方法应用Coulter-GENS system 2型全自动血液分析仪检测66例健康成人和33例地中海贫血(地贫)患者7项网织红细胞细胞参数.结果 (1)网织红细胞细胞多参数的正常参考值范围(男女混合)为:网织红细胞细胞百分比(RET%) 1.34±0.48(0.86%~1.82%);网织红细胞细胞绝对值(RET#)0.059±0.023(0.036~0.082);不成熟指数(IRF)0.3±0.04(0.26~0.34);高光散网织红细胞细胞绝对值(HLR%) 0.38±0.14(0.24%~0.52%);高光散网织红细胞细胞绝对值(HLR#) 0.017±0.007(0.01~0.024);体积参数-平均网织红细胞细胞体积(MRV)113±7(106~120);平均球红细胞体积(MSCV) 91±6(85~97).(2)地贫患者7项网织红细胞细胞参数中多项与正常人相比有显著性差异,其中RET% 、RET# 、HLR%和 HLR#明显增高(P<0.01),MRV、 MSCV明显下降(P<0.01),且变化程度与病情的严重程度有一定的关联性. 贫血程度较轻的血红蛋白病,网织红细胞各参数变化不大.结论网织红细胞细胞多参数测定提示,地中海贫血时患者网织红细胞细胞中未成熟细胞所占比例增加可能是导致'无效造血'的主要原因之一;网织红细胞细胞多参数分析能在一定程度上提示地中海贫血严重程度;在经典网织红细胞指标RET%、RET#基础上应用HLR%、 HLR# 和IRF,可以更细微、全面地描述网织红细胞的成熟度,从而更好地评价骨髓造血的功能.
Objective To investigate the reference value range of automated reticulocyte multiple parameters analysis in Chinese healthy people,and to discuss changes and clinical significances of these parameters in thalassemia patients. Methods Automated reticulocyte counting(7parameters) were measured in 66 healthy controls and in 33 thalassemia patients with Coulter-GENS system 2 automated blood cell counter. Results The reference ranges of automated reticulocyte multiple parameters analysis in normal healthy were: RET% 1.34±0.42(0.86%-1.82%);RET# 0.05 9±0.019(0.036- 0.082 );IRF 0.3±0.04(0.26-0.34);HLR% 0.38±0.12(0.24%-0.52%);HLR# 0.017±0.005(0.01-0.024);MRV 113±7(106-120);MSCV 91±6(85-97). There were significant differences between normol subjects and thalassemia patients in most of the parameters. RET%,RET#,HLR% and HLR# in thalassemia patients were significantly higher than those in healthy control, otherwise, MRV and MSCV were obviously decreased in thalassemia patients. There were some correlations between the changes of reticulocyte multiple parameters and the severity of patients.Conclusion The increased proportion of immature reticulocytes in thalassemia patients might be one of the main reasons of ineffective erythropoiesis. Automated reticulocyte multiple parameters analysis can give information about severity of thalassemia to some distance
