乳腺肿块的彩色多普勒能量图与病理对比观察

摘要 目的探讨乳腺肿块的彩色多普勒能量图与病理性质的相关性,达到提高超声对乳腺肿块的诊断符合率。方法对60例乳腺良、恶性肿块进行彩色多普勒超声检查,对检查结果与病理结果进行对照分析。结果病理诊断良性肿瘤34例(56.6%),乳腺恶性肿瘤26例(43.3%),其中浸润性导管癌18例(30.0%),髓样癌7例(11.7%),硬癌1例(1.6%)。彩色多普勒能量图显示乳腺良性肿瘤周边多数血流减少,肿块内部多数无血流信号,恶性肿瘤肿瘤周边及内部血流明显增多;乳腺恶性肿瘤彩色多普勒血流频谱形态常表现为升降波陡直,舒张期起始波在收缩期下降波中点以下,阻力指数及脉动指数增大;乳腺纤维腺瘤多普勒血流频谱形态则常表现升降波斜平,舒张期起始波在收缩期下降波中点以上,阻力指数及脉动指数正常。本组对乳腺肿块的诊断准确率为95%(57/60);对乳腺癌的敏感性为96.2%(25/26)。结论彩色多普勒血流频谱形态是鉴别乳腺良、恶性肿块的一项重要指标。 Objective To evaluate the correlation between color doppler power imaging and pathology of breast neoplasm, and to raise the coincidence rate of ultrasound diagnosis for breast neoplasm. Methods Sixty breast neoplasms were examined by color doppler ultrasonography. The comparison was studied between ultrasonographic and pathologic images. Results Thirty-four cases (56.6%) were benign breast neoplasms, consisted of 32 cases (51.7%) of breast fibroadenoma, 3 cases (3.3%) of, hyperplasia of mammary glands. Twenty-six cases (43.3 % ) were malignant breast neoplasms, consisted of 18 cases (30.0% ) of infiltrating ductal carcinoma,7 cases of medullary carcinoma, 1 case (1.6%) of mucinous carcinoma. Most blood flows besides benign breast neoplasms was detected to be decreased by color doppler power imaging, but no blood flow was detected in most benign breast neoplasms. Blood flows were detected to be increased besides and in malignant breast neoplasms. Color doppler blood stream spectrum forms of malignant breast neoplasms show Phenotype hoisting steep. Relaxing period initiation wave Systole descent wave to expect to drop count in wave following. Resistance index and pulsation index increase . Color doppler blood stream spectrum forms of Breast fibroadenoma show hoisting wave Oblique level. Diastole issue initiation wave in Systole descent wave king above. Resistance index and pulsation index are normal. The diagnostic accuracy rate of breast neoplasms by was 95 % (57/60), of which 1 case of breast cancer were misdiagnosed to be benign breast neoplasm, of which 2 cases of benign breast neoplasms was misdiagnosed to be breast cancers.The sensibility of color doppler ultrasonography for breast cancer was 96.2% (25/26). Conclusion Color doppler blood stream spectrum form is a important index in differential diagnosis of benign and malignant breast neoplasm.
